Effects of Comprehensive Combined Core Stability and Lumbar-Pelvic Stabilization Exercises on Fear of Movement, Depression, and Anxiety in Female Patients with Non-Specific Chronic Low Back Pain: A Randomized Controlled Single-Blind Clinical Trial

Abstract

Introduction: Chronic low back pain is one of the most common musculoskeletal disorders that affects patients' daily activities and physical functioning. The aim of this study was to investigate the effects of comprehensive combined core stability and lumbar-pelvic stabilization exercises on fear of movement, depression, and anxiety in patients with non-specific chronic low back pain.
Method: This was a randomized controlled single-blind clinical trial involving 40 women with non-specific chronic low back pain. Participants were randomly assigned to either the comprehensive combined core stability and lumbar-pelvic stabilization exercise group (n=20) or the control group (n=20). Assessment was conducted using the Tampa Scale for Kinesiophobia (TSK) and the Hospital Anxiety and Depression Scale (HADS). The experimental group underwent the comprehensive treatment protocol for eight weeks, with three sessions per week. One-way analysis of covariance (ANCOVA) was used to compare between-group differences, and paired t-test was employed to assess within-group changes.
Results: One-way analysis of covariance (ANCOVA) revealed a significant difference between the comprehensive combined core stability and lumbar-pelvic stabilization exercise group and the control group in reducing fear of movement and decreasing the severity of depression and anxiety, with a large effect size (P < 0.05). Additionally, within-group changes demonstrated that the eight-week comprehensive rehabilitation protocol resulted in a significant improvement in reducing fear of movement, depression, and anxiety (P < 0.05).
Conclusion: The results of the present study demonstrated that adding lumbar-pelvic stabilization exercises to core stability exercises was effective in reducing fear of movement and decreasing the severity of depression and anxiety in women with chronic low back pain. Physicians, physical therapists, and occupational therapists may consider incorporating lumbar-pelvic stabilization exercise rehabilitation programs as part of comprehensive treatment plans for patients with chronic low back pain.
