The Causal Relationship Between Alexithymia and Quality of Life in Patients with Chronic Pain: The Mediating Role of Pain Intensity

Background: Chronic pain, as a common problem, can affect patients' physical health, psychological well-being, and quality of life. Therefore, the present study aimed to investigate the causal relationship between alexithymia and quality of life, with the mediating role of pain intensity, in patients with chronic pain. Method: This study utilized a Structural Equation Modeling (SEM) approach. The statistical population of the present study comprised individuals aged 18 to 65 years with chronic pain who attended treatment clinics in the city of Kashan in the year 2022-2023. From among them, 200 individuals were selected using a convenience sampling method and based on the desired inclusion criteria.Data were collected using the World Health Organization Quality of Life-BREF (WHOQOL-BREF, 1996), the Toronto Alexithymia Scale (TAS-20; Bagby et al., 1994), and the Von Korff Pain Intensity Scale (1990). The collected data were analyzed using SPSS-AMOS 24 software. Results: Findings showed that the proposed research model demonstrated a good fit. The results indicated that the direct effect of alexithymia on quality of life (β = -0.625, p < 0.01), the direct effect of alexithymia on pain intensity (β = 0.273, p < 0.01), and the direct effect of pain intensity on quality of life (β = -0.147, p < 0.05) were significant. Furthermore, the results showed that the indirect effect of alexithymia on quality of life mediated by pain intensity was significant (β = -0.040, p < 0.05). Conclusion: Identifying variables related to chronic pain can guide interventions aimed at improving the quality of life for affected individuals.
