Health inequalities are among the controversial and challenging aspects of health systems, and one of the important goals of any health system is to reduce health care inequalities and their determinants. The Sustainable Development Goals by 2030 focused on reducing extreme poverty, increasing income, eliminating chronic hunger, reducing inequality in various dimensions of gender, education, access to health care, and other health and well-being indicators. Although the Millennium Development Goals have been achieved to some extent in many countries and made significant progress in many indicators. However, the situation of inequality in health and well-being indicators in Iran, especially in vulnerable populations, is still problematic and, given the country's economic situation and changes in the prices of medicines and basic goods in the future, it could set the country back from achieving the Millennium Development Goals. So far, numerous plans have been implemented in the health system with the aim of reducing health inequalities, increasing people's access to services, reducing out-of-pocket costs, and ensuring equity in health in the country, but the results of recent studies still show inequality in access, utilization of services, and health outcomes. Therefore, since achieving equity and eliminating health inequalities through the approach of social determinants of health has been mentioned in most of the upstream national programs and laws in Iran, in this policy recommendation we intend to examine strategies for reducing it in Iran.
