پیش‌آگهی درازمدت بیماری آرتریت روماتوئید: یک مطالعه کوهورت گذشته‌نگر چندمرکزی

سابقه و هدف: آرتریت روماتوئید شایع‌ترین بیماری التهابی سیستمیک مزمن و اتوایمیون است. با توجه به این‌که یکی از اهداف مهمّ درمان این بیماری رسیدن به رمیسیون می‌باشد، این مطالعه به‌منظور شناخت عوامل مرتبط با پیش‌آگهی این بیماری طراحی‌ شده تا درمانی متناسب با شدت آن انتخاب گردد.
مواد و روش‌ها: در این مطالعه، پرونده 433 بیمار مبتلا به آرتریت روماتوئید توسط چند روماتولوژیست و دستیاران آن‌ها در مراکز تبریز و کاشان مورد بررسی قرار گرفت و عوامل مؤثر بر پیش‌آگهی بیماران با استفاده از آزمون رگرسیون لجستیک مشخص شد.
نتایج: متغیر‌های طول مدت بیماری، DAS28 در اوّلین مراجعه، رژیم ‌درمانی و مصرف داروی TNF.inhibitors بر پیش‌آگهی بیماران تأثیر معناداری دارند (05/0P<) و با افزایش یک واحد (سال) در طول مدت بیماری، شانس پیش‌آگهی ضعیف در بیماران 117/1 برابر افزایش می‌یابد؛ شانس پیش‌آگهی ضعیف در بین بیمارانی که فعالیت بیماری در آن‌ها در ابتدای مراجعه شدید بوده است، 369/2 برابر بیمارانی است که فعالیت بیماری در آن‌ها متوسط بوده است؛ همچنین شانس پیش‌آگهی ضعیف در بین بیمارانی که رژیم درمانی آن‌ها تک‌دارویی بوده است، 346/9 برابر بیمارانی است که رژیم درمانی دودارویی داشته‌اند.
نتیجه‌گیری: افزایش طول مدت بیماری، شدیدبودن فعالیت بیماری در ابتدای مراجعه و استفاده از رژیم تک‌دارویی در شروع درمان، شانس پیش‌آگهی ضعیف در بیماران را افزایش می‌دهد.
