A case report of malignant hyperthermia during general anesthesia for proximal humeral fracture repair in a young patient

Background and Aim: Malignant Hyperthermia (MH) is a rare, life-threatening emergency that can be triggered in susceptible individuals by volatile anesthetic agents. This article reports a fatal case of this complication.
Case Presentation: The patient was a 19-year-old male who underwent general anesthesia with isoflurane for surgical repair of a proximal humeral fracture. Intraoperatively, a sudden increase in end-tidal carbon dioxide and a rise in core body temperature were observed. Upon the presumptive diagnosis of malignant hyperthermia, the anesthetic agents were discontinued and the surgery was halted. Immediate treatment, including the administration of dantrolene and cold saline lavage, was initiated. Despite these interventions, the patient experienced cardiac arrest and succumbed.
Conclusion: This case emphasizes the critical importance of vigilant monitoring during anesthesia, preparedness for the prompt diagnosis and management of MH, and the immediate availability of dantrolene. Enhancing awareness and training in emergency protocols for all operating room personnel is imperative.
