تأثیر برنامه‌های آموزشی مبتنی بر مدل خودمراقبتی اورم بر کیفیت زندگی بیماران: یک مرور سیستماتیک

زمینه و هدف: مدل خودمراقبتی اورم به عنوان یک چارچوب نظری قدرتمند، نقش مهمی در توانمندسازی بیماران ایفا می‌کند. با این حال، شواهد مربوط به تأثیر برنامه‌های آموزشی مبتنی بر این مدل بر کیفیت زندگی بیماران، پراکنده و نیازمند یکپارچه‌سازی است. این مرور سیستماتیک با هدف تعیین تأثیر این گونه برنامه‌های آموزشی بر کیفیت زندگی بیماران انجام شد.  
روش‌ها: در این مرور سیستماتیک، بازیابی مقالات انگلیسی و فارسی چاپ شده در بانک‌های اطلاعاتی PubMed،Scopus ، ProQuest، Cochrane، Web of Science، CINAHL و پایگاه‌های داخلی Magiran، SID، ISC، Iran Doc و همچنین موتور جست‌وجوی  Google scholarطی سال‌های ۲۰۲۴-۲۰۰۰ با کلمات کلیدی مدل خودمراقبتی اورم، کیفیت زندگی و معادل‌های MESH آنها با همه ترکیبات احتمالی با استفاده از عملگرهای AND یا OR انجام ‌شد. از بین ۱۸۵۴ مطالعه بازیابی‌شده، پس از حذف موارد تکراری و ارزیابی عنوان و چکیده، ۴۷ مطالعه که معیارهای ورود را دارا بودند، انتخاب و کیفیت روش‌شناختی آنها با استفاده از ابزار Joanna Briggs Institute (JBI) ارزیابی شد.
یافته‌ها: در نهایت ۴۷ مطالعه (۲۷ مطالعه از ایران و ۲۰ مطالعه از سایر کشورها) وارد مرور شدند. این مطالعات بر روی بیماران مبتلا به انواع شرایط مزمن (مانند سرطان، دیابت، بیماری‌های قلبی-عروقی، نارسایی کلیوی) در جمعیت‌های بزرگسال و سالمند متمرکز بودند. مداخلات آموزشی بر اساس سه رویکرد حمایتی-آموزشی، جبرانی نسبی و جبرانی کامل مدل اورم طراحی شده بودند. در ۴۶ مطالعه (۹۸٪)، اجرای برنامه آموزشی منجر به بهبود معنادار آماری در نمره کلی کیفیت زندگی شده بود. علاوه بر این، بهبود در شاخص‌های ثانویه مانند خودکارآمدی، اضطراب، افسردگی و رفتارهای خودمراقبتی نیز گزارش شده بود.
نتیجه‌گیری: یافته‌ها به طور قوی از کارایی مدل خودمراقبتی اورم به عنوان یک چارچوب مؤثر برای طراحی برنامه‌های آموزشی به منظور ارتقای کیفیت زندگی بیماران حمایت می‌کند. موفقیت این مداخلات مستلزم بومی‌سازی و تطبیق آنها با زمینه فرهنگی-اجتماعی جامعه هدف است. بر اساس این مرور، طراحی مداخلات مبتنی بر نیازسنجی دقیق، ارائه آموزش‌های فردمحور و اجرای پیگیری‌های منظم پس از آموزش توصیه می‌شود. اگرچه نقاط قوت این مرور، بررسی طیف وسیعی از بیماری‌ها است، اما محدودیت‌هایی مانند ناهمگونی روش‌شناختی مطالعات و تمرکز جغرافیایی بر آسیا باید در تعمیم‌دهی نتایج مورد ملاحظه قرار گیرد.
