بررسی اپیدمیولوژی بیماران مبتلا به ضربه های چشمی مراجعه کننده به مرکز چشم پزشکی شهر کاشان طی 6 ماه اول سال 1374

<p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> با توجه به تناقضات موجود در ویژگی های اپیدمیولوژی بیماران مبتلا به تروماهای چشمی از مراکز مختلف دنیا، به منظور تعیین میزان شیوع و خصوصیات مبتلایان به ضربه های چشمی این مطالعه بر روی بیمارانی که طی 6 ماه اول سال 1374 به اورژانس بیمارستان متینی شهر کاشان مراجعه نمودند انجام گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> مطالعه به روش توصیفی صورت پذیرفت. بیماران با تشخیص ضربه چشم و اطراف آن وارد این مطالعه شدند. خصوصیات فردی و معاینات چشمی آن ها در پرسش نامه ای ثبت گردید و نوع ضربه تیز، کند، شیمیایی، حرارتی یا تشعشعی، وسیله وارد کننده ضربه، موارد بستری، مدت و علت بستری و درمان های مربوط ثبت و نتایج آن ها با آمار توصیفی ارایه شد. </p><p style="DIRECTION: rtl" align="justify"><strong>یافته ها:</strong> از مجموع 1261 نفر مورد بررسی 85.3% مرد و 14.7% زن بودند (نسبت مرد به زن 5.8 به یک بود) دهه سوم با 32% موارد و متوسط سنی 24 سال شایع ترین محدوده سنی شیوع بیماری بود. فراوانی ضربه در ماه های مختلف تفاوت چشمگیری نداشت. شایع ترین مشاغل در معرض ضربه های چشمی، محصلین، کارگران، جوشکارها، کودکان و زنان خانه دار بودند. شایع ترین نوع ضربه، جسم خارجی (55% موارد) می باشد و در درجه بعدی ضربه های کند (25%) و ضربه های نافذ و اشعه ماورا بنفش هر کدام 7% و آسیب شیمیایی 2% بودند. اعضای چشمی که بیش از همه در ضربه ها صدمه دیده بودند به ترتیب: قرنیه (45%) و ملتحمه (33%) هستند. شایع ترین وسایل و راه های ایجاد آسیب در چشم در انواع پنج گانه آسیب ها، پلیسه، میله فلزی، چاقو، افتادن، وسایل نقلیه، آب داغ، آهک و شوینده ها بوده است. چهار درصد بیماران مبتلا به ضربه های چشمی نیاز به بستری پیدا کردند و 2.4% از آن ها مورد عمل جراحی قرار گرفتند. میانگین مدت بستری بیماران 5 روز بود. بیشترین کاهش دید در بین بستری شدگان در موارد ضربه های نافذ (44% دید شمارش انگشتان یا کمتر) و سپس در ضربه های کند (29% دید شمارش انگشتان یا کمتر) داشتند. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> ضربه های چشمی در مردان تقریبا 6 برابر شایع تر است اما مواردی که زنان و اطفال را گرفتار می کند شدیدتر بوده، پیشگیری و مراقبت بیشتری را طلب می کند. ضربه های نافذ آسیب های شدیدتر از ضربه های کند بوجود می آورند و نیاز به مراقبت بیشتری دارند. با توجه به آن که اغلب ضربه های چشمی صدمه خفیفی وارد می کنند با آموزش درمان موارد شایع به کارآموزان و کارورزان می توان بیش از 90% آن ها را درمان نمود. این بررسی پس از اعمال روش های حفاظتی و پیشگیرانه بخصوص در سطح کارگاه ها و کودکان مجددا تکرار و اهمیت روش های مزبور تاکید می کند. </p>
<p align="justify"><strong>History and Objectives:</strong> Due to discrepancies in the specificities of reported cases of eye trauma and in order to assess the prevalence and characteristics of patients affected with ocular injuries, the present study was conducted on patients referred to the emergency department of Matini hospital of Kashan in 1995. </p><p align="justify"><strong>Materials and Methods:</strong> A descriptive investigation on patients with eye trauma was carried out. Patient’s personal records, eye examination details, type of trauma (Sharp object or heavy blow, chemical, thermal trauma, radiation), type of object involved, duration and reason for hospitalization and treatment was recorded. On the basis of the data gathered statistical analysis was carried out. </p><p align="justify"><strong>Results:</strong> 1261 patient was examined (85.3% male and 14.7% female, ratio of male to female was 5.8 to 1). Most patients were in their third decade of life and most prevalent age was 24. The prevalence of eye trauma was even through out the year. Students, laborers, welders, children and housewives were most frequent groups referred to the emergency department. The most prevalent type of trauma was caused by presence of an object in the eye (55%) followed by mild trauma (25%), penetrating eye injuries and UV radiation (7% each) and chemical injuries (2%). Cornea (45%) and conjunctiva (33%) were areas most involved in the ocular injuries. Iron particles, steel rod, knife, falling, motor vehicles accidents, hot water, bicarbonate and detergents were the most prevalent agents involved in the ocular injuries. Four percents of patients required hospitalization and 2.4% required surgical procedures average stay in the hospital was 5 days. Penetrating eye injuries (44% fingers counting ability or less) and mild eye injuries (29% fingers counting ability or less) were the most probable cause of reduced visual ability. </p><p align="justify"><strong>Conclusion:</strong> Eye injuries among male patients were 6 times more frequent than their female counterparts, however ocular trauma in women and children were more severe in nature and therefore more attention and preventive measures are required. Since most ocular injuries result in mild trauma, giving therapeutic instruction, students and interns can deal with 90% of such cases successfully. The present study can be repeated after preventive instructions are given to children and factories. </p>
