Ovarian tissue cryopreservation (OTC) remains the only established fertility preservation option for prepubertal girls and patients requiring urgent treatment initiation. In Iran, systemic barriers limit equitable access to this essential service.</span><br> <span style="unicode-bidi:embed">Policy Options: the three key policy options include adopting and promulgating the &ldquo;National Protocol on Ethics and Reproductive Rights in Cancer,&rdquo; increasing knowledge, quality, and public awareness, and integrating fertility preservation services into the &ldquo;National Cancer Care Network.<span dir="RTL" lang="AR-SA"><span style="font-family:"B Mitra""></span></span></span><br> <span style="unicode-bidi:embed">Recommendation: Establishing a national network of public hospital-based ovarian tissue biobanks supported by centralized coordination and governance mechanisms.
