بررسی پیش آگهی کوتاه مدت و میان مدت در مبتلایان سکته مغزی کاشان طی سال های 76-1375

<p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> سکته مغزی در کشورهای پیشرفته سومین علت مرگ و میر بعد از بیماری های قلبی و سرطان و در کشورهای در حال پیشرفت دومین علت مرگ و میر بعد از بیماری های قلبی می باشد. بیش از 50% بیماران مبتلا به سکته مغزی (CVA) یک فشار خون بالای شریانی داشتند. سکته مغزی در بیماران دیابتی، 6-2 برابر شایع تر از افراد غیر دیابتی و میزان مرگ و میر افراد دیابتی دو برابر بیماران غیر دیابتی می باشد. با توجه به اهمیت فشار خون بالا و دیابت و مشخص نبودن پیش آگهی بیماران مبتلا به CVA در شهر کاشان مطالعه زیر جهت تعیین پیش آگهی کوتاه مدت و میان مدت بیماران مبتلا به CVA در سال 76-1375 انجام گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> پژوهش حاضر به روش توصیفی بر روی کلیه بیماران مبتلا به CVA بستری در بخش داخلی بیمارستان شهید بهشتی و مراجعه به مطب شخصی در سال 76-1375 صورت پذیرفت و اطلاعاتی شامل سابقه دیابت، فشار خون، اختلالات تکلم، سطح هوشیاری، حرکتی، نوع سکته، سن، جنس این بیماران در بدو ورود، 10 روز بعد یا هنگام ترخیص و در نهایت سه ماه بعد از مراجعه جمع آوری شد و با توجه به وضعیت بهبودی کامل، ناقص، فوت مورد ارزیابی قرار گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>یافته ها:</strong> از 410 بیمار مورد بررسی، 310 نفر فشار خون، 60 نفر دیابتی و 40 نفر نه فشار خون و نه دیابت داشتند. پیش آگهی بیماران در کوتاه مدت در صورتی که مبتلا به فشار خون باشند، بد خواهد بود به گونه ای که در بیمارانی که فشار خون ندارند، شانس بهبودی کامل 4 برابر افزایش می یابد و در میان مدت شانس بهبودی کامل در همین بیماران که فشار خون ندارند، 3 برابر افزایش پیدا می کند و همچنین پیش آگهی بیماران در کوتاه مدت در صورتی که مبتلا به دیابت باشند، بد خواهد بود به گونه ای که در بیمارانی که دیابت ندارند، شانس بهبودی کامل دو برابر و در میان مدت شانس بهبودی کامل در همین بیماران که دیابت ندارند سه برابر افزایش می یابد. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> در این مطالعه هم پیش آگهی کوتاه مدت و هم میان مدت در بیماران فشار خونی و دیابتی بدتر از بیماران مبتلا به CVA غیر فشار خونی و دیابتی است. از این رو، بررسی بیشتر و جلوگیری و درمان کامل بیماران دیابتی و فشار خونی می تواند تا حد زیادی از بروز CVA و عوارض بعد از آن نسبت به بیمارانی که چنین سابقه ای را ندارند بکاهد. </p>
<p align="justify"><strong>History and Objectives:</strong> Cerebrovascular accident (CVA) is the third most common cause of death after cardiac and malignant diseases in developed countries, while in developing countries, CVA is the second only after cardiac diseases. More than 50% of patients with CVA have arterial hypertension. CVA is two to six times more common in diabetics than non-diabetics and the mortality rate is twice in diabetics compared with non-diabetic individuals. Due to the importance of hypertension and diabetes and the unknown prognosis of patients with CVA in Kashan, this study was performed in 1375 and 1376 in order to define the short and intermediate-term prognosis of patients with CVA. </p><p align="justify"><strong>Materials and Methods:</strong> This study was performed by descriptive method on all hospitalized patients with CVA at the ward of internal medicine of Shaheed Beheshti hospital and patients referring to private clinic as outpatients in 1375 and 1376. Data including history of diabetes, hypertension, speech disorders, level of consciousness, motor disturbance, type of infarction, age and sex of the patients were collected at the time of diagnosis, ten days after diagnosis or on discharge and finally three months later. The outcome was considered as complete improvement, partial improvement or death. </p><p align="justify"><strong>Results:</strong> From the 410 patients under study, 310 individuals had hypertension, 60 were diabetic and 40 patients had neither hypertension nor diabetes. Short-term prognosis is poor in patients with hypertension as the rate of complete improvement increases by fourfold in short-term and by threefold in intermediate-term. Short-term prognosis is also poor in diabetics as the chance of complete improvement in short and intermediate-term increases by twofold and threefold in non-diabetic, respectively. </p><p align="justify"><strong>Conclusion:</strong> Short and intermediate-term prognosis were worse in hypertensive and diabetic patients than non-hypertensive and non-diabetic individuals. Therefore further study prevention and complete treatment of diabetic and hypertensive patients can significantly reduce the risk of CVA and its consequent complications. </p>
