آگاهی و رویکرد پزشکان متخصص کودکان از مسایل مطرح شده در دستورالعمل های WHO در مورد گاستروآنتریت حاد

<p><strong>سابقه و هدف:</strong> با عنایت به نگرانی از نحوه بکارگیری دستورالعمل های سازمان جهانی بهداشت (WHO) و نیز اهمیت اجرای آن ها، عوارض شناخته شده عدم بکارگیری این دستورالعمل ها، عدم اطلاع از وضعیت موجود و به منظور تعیین رویکرد پزشکان متخصص کودکان از مسایل مطرح شده در دستورالعمل WHO در مورد گاستروآنتریت حاد، این تحقیق در نزد پزشکان متخصص کودکان شرکت کننده در همایش سالانه انجمن پزشکان کودکان ایران در سال 1378 انجام گرفت. </p><p><strong>مواد و روش ها:</strong> تحقق حاضر به روش توصیفی از نوع مقطعی نزد 95 نفر از پزشکانی که بیش از 10 سال از زمان فارغ التحصیلی آنان گذشته بود و به طور تصادفی انتخاب گردیده بودند، انجام گرفت. پرسش نامه ای با 17 بند در اختیار آنان قرار گرفت که ضمن ثبت خصوصیات فردی، برای سنجش دانش نظری و نحوه تصمیم گیری درمان، با طرح سوالاتی و نیز گزارش مواردی از گاستروآنتریت، آگاهی و رویکرد آنان از دستورالعمل های WHO در مورد محلول های مایع درمانی خوراکی (ORS) و روش تغذیه در مرحله حاد گاستروآنتریت بررسی و ثبت گردید و سپس پاسخ های آنان براساس دستورالعمل های WHO مورد ارزیابی قرار گرفت. </p><p><strong>یافته ها:</strong> از 95 نمونه انتخابی، 87 نفر به تمام سوالات مطرح شده پاسخ دادند. سن پزشکان 17±44 سال و سابقه اشتغال آنان 6±17 سال بود. هفتاد و سه درصد پزشکان آگاهی مناسب داشتند ولی 60% پزشکان دستورالعمل های WHO را بکار می بردند. شصت و نه درصد پزشکان دارای دانش بکارگیری ORS بودند و در مقابل 83% پزشکان رویکرد مناسب بکارگیری ORS را در درمان داشتند. سی و یک درصد پزشکان به لحاظ نظری و 37% پزشکان از نظر عملی در مورد ادامه تغذیه در طول مدت حاد اسهال، عملکرد متناقض با دستورالعمل های WHO داشتند. </p><p><strong>نتیجه گیری:</strong> کیفیت بکارگیری دستورالعمل های WHO در مورد ادامه تغذیه در مرحله حاد گاستروآنتریت نازل است و تحقیقی برای بررسی علل آن و اقدامات لازم برای ارتقای آن پیشنهاد می گردد. </p>
<p><strong>History and Objectives:</strong> Considering the concern for ways of application of WHO guidelines and the problems as a result of not executing these items and for determination of tendency of pediatricians for applying the suggested protocols for acute gastroenteritis, this study was carried out on pediatricians who participated in a yearly congress in 1999. </p><p><strong>Materials and Methods:</strong> The descriptive and cross-sectional strategy of this study was randomly performed on 95 pediatricians who had graduated more than 10 years. In this regard, a 19-item questionnaire was prepared containing questions on personal characteristics, knowledge, information on gastroenteritis, use of ORS and feeding pattern for acute gastroenteritis and their answers were analyzed according to WHO guidelines. </p><p><strong>Results:</strong> Out of 95 selected cases, 87 ones answered all of the questions and their age was 44±17 years and a working record of 17±6. In addition, 73% of them had enough information, but only 60% of them applied WHO guidelines. Meanwhile, 69% of them had enough information regarding use of ORS and 83% had an appropriate tendency for using ORS. Theoretically, 31% of these specialists and practically, 37% of them showed controversial views regarding the continuous use of foodstuffs during the period of acute diarrhea as recommended by WHO. </p><p><strong>Conclusion:</strong> Application of WHO guidelines have not been executed for continuation of feeding during the acute phase of gastroenteritis. Therefore, it is recommended to explore the causative factors and to perform the necessary procedures for quality upgrading. </p>
