fa بررسی عوارض مادری و جنینی ناشی از تروماهای حاملگی در کاشان طی سال های87 – 1384 عمومى General پژوهشي Research <p align="justify"><strong>سابقه ­ و هدف:</strong> با افزایش میزان بروز تروماهای غیر نافذ و نافذ، رخداد این حوادث در حاملگی رو به افزایش است. این تروماها می­توانند منجر به عوارض مادری و جنینی شوند. با توجه به شیوع و اهمیت تروما و تناقضاتی که در زمینه عوارض و علل آن وجود دارد، این مطالعه به منظور تعیین عوارض مادری و جنینی ناشی از تروما در حاملگی در سال­های 87 – 1384 در بیمارستان های آموزشی کاشان صورت گرفت. </p><p align="justify"><strong>مواد و ­روش­ها:</strong> این پژوهش با طراحی توصیفی در 5500 مورد زایمان انجام شده صورت گرفت. کلیه 60 مورد مادر باردار که به علت تروما در این مراکز بستری شده بودند، از نظر مشخصات فردی، نوع تروما، محل آسیب، عوارض مادری و جنینی بر اساس معاینه بالینی مورد مطالعه قرار گرفتند و موارد فوق تعیین و تثبیت شد. یافته­ها به صورت آمار توصیفی و جداول توزیع فراوانی گزارش گردید. </p><p align="justify"><strong>نتایج:</strong> از 5500 زایمان انجام شده،60 نفر (1/1 درصد) به علت تروما بستری شده بودند. 55 نفر(6/91 درصـد) در گـروه سنـی 18 تا 35 سـال قــرار داشتنـد. از نظـر سـن حاملگـی، 15 نفـر(25 درصد) زیر 20 هفته،40 نفر (6/66 درصد) بین 36 - 20 هفته و 5 نفر (9 درصد) 37 هفته به بالا بودند. نوع تروما در 58 نفر (7/96 درصد) غیرنافذ و علت تروما در (7/61 درصد) افراد تصادفات رانندگی بوده است. 5 نفر (3/8 درصد) دچار عوارض مادری از جمله زایمان زودرس، نیاز به سزارین و دکولمان جفت و 8 نفر (3/13 درصد)، دچار عوارض جنینی نوزادی شامل سقط، افت ضربان قلب، تولد نوزاد نارس و مرگ داخل رحمی شدند . </p><p align="justify"><strong>نتیجه­گیری:</strong> شیوع تروماهای حاملگی در این مطالعه بالاتر از مطالعات مشابه در سایر کشورها است و با افزایش عوارض مادری و جنینی همراه می باشد. با توجه به اهمیت موضوع و به منظور کاهش خطرات ناشی از تروما، اقدام در جهت افزایش سطح ایمنی زنان باردار توصیه می شود. </p> <p align="justify"><strong>Background</strong>: With the advancing rate of the blunt and non-blunt traumas, the occurrence of such traumas in pregnancy increases. Such traumas could result in mother and fetus complications. Considering the prevalence and the importance of trauma and the controversy seen on the causes and complications, the present study was designed to evaluate the mother and fetus complications induced by pregnancy traumas in Kashan educational hospitals during 1384-1387. </p><p align="justify"><strong>Materials and Methods: </strong>This descriptive study was carried out on 5500 deliveries. All pregnant (n=60) hospitalized for traumatic reasons in the center were studied for personal data, type of the trauma, site of injury, mother and fetus complications based on physical examination. Findings was reported as descriptive statistics and frequency distribution tables. </p><p align="justify"><strong>Results: </strong>Sixty (1.1%) out of the 5500 delivery cases <strong></strong>were hospitalized for traumatic reasons. Fifty-five (91.6%) were in age range of 18-35 years. In terms of gestational age 15 (25%) cases were < 20 weeks 40 (66.6%) between 20-36 weeks and 5(9%) >20 weeks. The type of trauma in 58 (96.7%) was non-blunt and the reason in 61.7% of cases was accidents. Five cases had mother complications (e.g. premature delivery, need for cesarean and placental decollement) and 8 (13.3%) cases had fetus complications (abortion, heart rate fall, premature delivery and in-utero death). <strong></strong></p><p align="justify"><strong>Conclusion: </strong>Based on our study the prevalence of delivery traumas are higher than the similar studies in other countries and are concurrent with the increasing mother and fetus complications.
