بررسی سرواپیدمیولوژی تب مالت انسانی در شهرستان کاشان در سال 1375

<p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> با توجه به اندمیک بودن تب مالت و وجود گزارش های متعددی از موارد انسانی و حیوانی آن در ایران، میزان شیوع تب مالت انسانی در شهرستان کاشان در سال 75 تعیین گردید. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> 2376 نفر که به روش نمونه گیری تصادفی چند مرحله ای انتخاب شده بودند در یک مطالعه توصیفی مورد بررسی قرار گرفتند. روش مطالعه شامل مصاحبه و تکمیل پرسش نامه ای حاوی ویژگی های افراد مورد بررسی، شغل، سابقه تماس با دام، مصرف لبنیات غیر پاستوریزه و پنیر تازه بود. مقدار 5cc خون از افراد مورد مطالعه گرفته و جهت جداسازی سرم و انجام تست های سرولوژی رزبنگال و رایت به آزمایشگاه مرکزی کاشان ارسال گردید. </p><p style="DIRECTION: rtl" align="justify"><strong>نتایج: </strong>تحلیل نتایج با روش آماری فاصله اطمینان (Confidence Interval) انجام گرفت. بیست و دو مورد از 2376 نفر (0.9%) دارای آنتی بادی تب مالت بودند، از این تعداد 17 نفر (77.3%) مرد و 5 مورد (22.7%) زن بودند میانگین سنی افراد آلوده 15.9±32.7 سال و سن 72.7% از آن ها کمتر از 40 سال بود. شانزده مورد (72.7%) از موارد مثبت در گروه شغلی پرخطر شامل پرسنل دامپزشکی، قصاب، سلاخ، دامدار و کشاورز و 60 مورد (27.3%) از گروه شغلی کم خطر بودند. نوزده مورد (86.4%) از افراد مثبت با دام تماس داشته، 18 مورد (81.1%) پنیر تازه استفاده کرده بودند و 21 نفر (95.5%) سابقه مصرف لبنیات غیر پاستوریزه داشتند. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> شیوع تب مالت در کاشان 0.9% می باشد، این بیماری در مردان شایعتر و سابقه مصرف لبنیات غیر پاستوریزه، پنیر تازه و تماس با دام از جمله فاکتورهای خطر ابتلا به آن می باشد. </p>
<p align="justify"><strong>History and Objectives:</strong> Brucellosis is endemic in Iran and many cases have been reported in humans and animals. Therefore, this study was performed in 1996 to determine the prevalence of human brucellosis in Kashan. </p><p align="justify"><strong>Materials and Methods:</strong> A total of 2376 individuals were chosen for this study by multistage random sampling. Data was collected through interviews and questionnaires to record personal information, occupation, history of contact with livestock and use of non-pasteurized dairy products and fresh cheese. In each case, 5 milliliters of blood was drawn and sent to Kashan’s Central Laboratory for serum separation and Rose-Bengal and Wright serologic tests. Results were analyzed by calculating the statistic confidence interval. </p><p align="justify"><strong>Results:</strong> In the 2376 cases studied, 22 (0.9%) had antibodies for Malt fever, 17 of item (77.3%) being male and the other 5 (22.7%) female. Age mean and standard deviation of those infected was 32.7±15.9 years, 72.7% of them being under the age of 40. Sixteen positive cases (72.7%) had high-risk occupations. 19 positive cases (86.4%) had contact with livestock, 18 (81.8%) had used fresh cheese and 21 (95.5%) had used non-pasteurized dairy products. </p><p align="justify"><strong>Conclusion:</strong> We therefore determined the prevalence of Malt fever in Kashan to be 0.9%. The disease is more prevalent in males and a history of non-pasteurized dairy use, use of fresh cheese and contact with livestock are of the risk factors for this infection. </p>
