بررسی رابطه سوء هاضمه تغذیه با اسهال پایدار
A study on the relationship between malnutrition and persistent diarrhea

<p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> نظر به شیوع اسهال های پایدار و عوارض ناشی از آن و نظر به تناقضات موجود در مورد عوامل بوجود آورنده بیماری و به منظور تعیین رابطه سوتغذیه با اسهال پایدار این تحقیق بر روی مراجعین به بیمارستان شهید بهشتی کاشان طی سال های 74-1370 انجام گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> تحقیق به روش مورد-شاهدی Case-Control روی 100 کودک مبتلا به اسهال پایدار (گروه مورد) و 200 کودک مبتلا به اسهال حاد (گروه شاهد) انجام گرفت. هر کودکی که اسهال او به طور ناگهانی شروع و بیشتر از دو هفته و کمتر از 4 هفته طول کشیده به عنوان اسهال پایدار تلقی و کودکان گروه مورد و شاهد به لحاظ جنس، مراجعه به یک بیمارستان در یک زمان و نیز ابتلا به اسهال و سایر عوامل تاثیرگذار روی اسهال پایدار با هم مشابه سازی (Matching) شدند و سوتغذیه با شاخص وزن کمتر از 90% وزن طبیعی مورد بررسی قرار گرفت. نقش سوتغذیه در اسهال پایدار محاسبه و خطر ابتلای نسبی (O.R) و فاصله اطمینان آن (C.I) تعیین گردید. </p><p style="DIRECTION: rtl" align="justify"><strong>یافته ها:</strong> 10% کودکان با اسهال حاد و 55% کودکان با اسهال پایدار مبتلا به سوتغذیه بودند (P<0.0001). سو تغذیه 11 برابر شانس ابتلا به اسهال پایدار را افزایش می دهد. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> با توجه به نقش بسیار زیاد سوتغذیه در بروز اسهال پایدار توصیه می شود که مساله سوتغذیه کودکان و علل بوجود آورنده آن مورد توجه قرار گیرد و تحقیقات گسترده تری در این زمینه توصیه می گردد. </p>
<p align="justify"><strong>History and Objectives:</strong> Due to the prevalence of persistent diarrhea and its complications and the controversies regarding its etiology, the present study was performed in patients referred to Kashan’s Shaheed Beheshti hospital in 1989-94 to determine the relationship between malnutrition and persistent diarrhea. </p><p align="justify"><strong>Materials and Methods:</strong> In this case-control study, 100 children with persistent diarrhea were selected as the case group and 200 children with acute diarrhea were chosen as controls. And child with a sudden inset of diarrhea lasting more than two weeks and less than four weeks was considered a case of persistent diarrhea. The cases and controls were matched based in sex, time of referral to the hospital, onset of diarrhea and other factors influencing persistant diarrhea. Malnutrition was defined as a weight index of less than 90% of the normal weight. The role of malnutrition in persistent diarrhea was determined by calculating the relative risk and confidence interval. </p><p align="justify"><strong>Results:</strong> 10% of the children with acute diarrhea and 55% of those with persistent diarrhea had malnutrition (P<0.001). The risk of developing persistant diarrhea was 11 times greater in the presence of malnutrition. ]</p><p align="justify"><strong>Conclusion:</strong> Due to the remarkable role of malnutrition in the incidence of persistent diarrhea, we suggest more attention be paid to the problem of malnutrition in children and further studies be performed to identify its causes. </p>
