علل خونریزی گوارشی فوقانی در کودکان مراجعه کننده به مرکز طبی کودکان طی سال های 75-1374

<p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> خونریزی گوارشی در هر سنی مخاطره آمیز بوده و مشخص کردن علت و کانون خونریزی از نظر درمان و پیش آگهی اساسی می باشد. با توجه به گزارش های متفاوت از علل مراجعه، علل و محل خونریزی، این تحقیق به منظور تعیین علل خونریزی گوارشی فوقانی در کودکان مراجعه کننده به مرکز طبی کودکان طی سال های 75-1374 انجام گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> پژوهش حاضر با روش توصیفی (Descriptive study) روی کودکانی که با شکایت خونریزی گوارشی مراجعه نمودند صورت پذیرفت و برای تمام آن ها بدون بیهوشی، آندوسکوپی با دید مستقیم به وسیله فلوی گوارشی اطفال به عمل آمد. خصوصیات کودکان، علت مراجعه، محل یا محل ها و علل خونریزی بررسی و ثبت گردید. </p><p style="DIRECTION: rtl" align="justify"><strong>یافته ها:</strong> از 166 کودک مورد بررسی در سنین 3.6±6.7 سال، 49% دختر بودند که 71% به علت هماتمز، 16% ملنا، 11% هماتمز و ملنا و 2% هماتوشیزیا مراجعه کردند. علت خونریزی در 90% مشخص و در 10% آندوسکوپی قسمت فوقانی گوارش طبیعی بود. گاستریت در 70%، دئودنیت 27% و واریس مری در 22% مهم ترین علت خونریزی می باشد و در 48 مورد علل مشترک خونریزی مشاهده گردید که بیش از یک سوم (1/3) آن ها گاستریت توام با دئودنیت بود. حدود 65% افراد مبتلا به زخم معده و اثنی عشر با هماتمز مراجعه نمودند. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> شایع ترین علت خونریزی در کودکان گاستریت و زخم ها از نوع پپتیک ثانوی بودند. انجام تحقیقات تحلیلی برای تعیین علل بروز این عارضه و تحقیقات تجربی برای درمان و پیشگیری از این بیماری توصیه می گردد. </p>
<p align="justify"><strong>History and Objectives:</strong> Gastrointestinal bleeding is life threatening at any given age. It is essential to determine the cause and location of such bleeding. The present study was undertaking in order to determine the referral justification and location of bleeding on patients referred to the Children Medical Center in 1995-1996, since the discrepancies on the reasons of referral has reported lately. </p><p align="justify"><strong>Materials and Methods:</strong> A descriptive study on children with gastrointestinal bleeding was carried. Endoscopy without anesthesia with direct vision was performed. Children’s personal information, reason for referral, location(s) and cause of bleeding was recorded. </p><p align="justify"><strong>Results:</strong> From 166 children under study (Mean age 6.7±3.6, 49% female), hematemesis (71%), melena (16%), hematemesis and melena (11%), hematochezia (2%) were the most prevalent reason of referral. The cause of bleeding in 90% of cases was determined and the 10% of patients had normal lower intestinal endoscopic pattern. Gastritis (70%), duodenitis (27%) and esophagus varicosis (22%) were the major cause of bleeding. In 48% of cases two or more of causes of bleeding was found where in one third of the cases, gastritis occurred along with duodenitis. In 65% of patients suffering from gastric and duodenal ulcerations, hematemesis was the major reason for referral. </p><p align="justify"><strong>Conclusion:</strong> The most prevalent cause of bleeding among children was gastritis and secondary peptic ulcers. Analytical investigations as to the underlying cause of such lesions and needed. Experimental clinical trials for therapy and prevention of such conditions are recommended. </p>
