تاثیر عوامل ضد میکروبی و تعیین MIC آنها در اشریشیاکلی های جدا شده از عفونت مجاری ادراری

<p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> نظر به شیوع عفونت مجاری ادراری و اهمیت آن از نظر درمان و مصرف نادرست آنتی بیوتیک ها در این مورد که منجر به افزایش بروز مقاومت آنتی بیوتیکی در بین عوامل اتیولوژیک بیماری از جمله اشریشیاکلی که حدود 90-80% از این عفونت را باعث می گردد، این تحقیق به منظور تعیین مقاومت آنتی بیوتیکی نسبت به عوامل ضد میکروبی رایج بر روی 50 سوش اشریشیاکلی جدا شده از عفونت مجاری ادراری در اصفهان طی تابستان 1377 انجام گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> پژوهش حاضر به روش توصیفی (Descriptive) صورت پذیرفت. از بیماران مراجعه کننده به آزمایشگاه های سطح شهر و بیمارستان های مختلف پس از گرفتن نمونه و تایید عفونت با روش های استاندارد، در صورت جدایی باکتری، آن را به آزمایشگاه میکروب شناسی دانشکده پزشکی اصفهان منتقل و پس از انجام آزمایش های دقیق، باکتری را با روش های استاندارد تشخیص و سپس آن را روی محیط های کشت مولر هینتون دارای رقت هایی از 0.25 میکروگرم در میلی لیتر تا 128 میکروگرم در میلی لیتر از 13 آنتی بیوتیک رایج، پلیت ها را انکوبه گذاری کرده و MIC آن ها به صورت دقیق مشخص گردید. </p><p style="DIRECTION: rtl" align="justify"><strong>یافته ها:</strong> از 50 سوش اشریشیاکلی مورد مطالعه به ترتیب 72، 64، 58، 56، 52، 52، 38، 34، 32، 24، 22، 20 و 18 درصد از آن ها به آمپی سیلین، کربنی سیلین، تتراسیکلین، کلرامفنیکل، سفوکسی تین، سفالوتین، نیتروفورانتوئین، سیپروفلوکساسین، آمیکاسین، جنتامایسین، کوتریموکسازول، کانامایسین و اسید نالیدیکسیک مقاوم بودند. موارد فوق دارای MIC بزرگتر از مقایسه با سوش اشریشیاکلی استاندارد و حساس E.coli ATcc 25922 بودند. همه سوش های جدا شده multiresistance بودند. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> با توجه به افزایش سوش های مقاوم اشریشیاکلی در عفونت مجاری ادراری و بالا بودن میزان آن در مقایسه با کشورهای پیشرفته، جلوگیری از مصرف خودسرانه آنتی بیوتیک توسط مردم با بالا بردن آگاهی های عمومی و همچنین تجویز آن ها بر مبنای آنتی بیوگرام دقیق آزمایشگاه توسط پزشکان توصیه می گردد. </p>
<p align="justify"><strong>History and Objectives:</strong> Due to prevalence of urinary tract infections and due to the importance of therapy and misuse of antibiotics which leads to augmented antibiotics resistance and since E.coli is the causative agent in 80 to 90% of cases, the present study is undertaken in order to determine the resistance of antibiotics by 50 different E.coli species separated from urine samples from city of Isfahan in 1998. </p><p align="justify"><strong>Materials and Methods:</strong> A descriptive study on patients referred to clinical diagnostic laboratories and hospitals of Isfahan was carried out. After confirmation of infection, the infectious agents were transferred to the department of microbiology of Isfahan Medical University. Classic microbiological techniques were used to identify the cells. Their resistance to the 13 different antibiotics (From 0.25 µg/ml to 128 µg/ml concentrations) and their MIC’s were determined. </p><p align="justify"><strong>Results:</strong> From 50 different E.coli species 72%, 64%, 58%, 56%, 52%, 52%, 38%, 34%, 32%, 24%, 22%, 20% and 18% were resistant to ampicillin, carbinicyllin, tetracycline, chloramphenicol, cefoxitin, cephalothin, nitrofurantoin, ciprofloxacin, amikacin, gentamicin, co-trimoxazole, kanamycin, cefoxitin and nalidixic acid respectively. MIC for the above-mentioned cell were higher than the standard sensitive E.coli (E.coli ATCC 25922). All the isolated species were multiresistant. </p><p align="justify"><strong>Conclusion:</strong> In light of higher rate of E.coli resistance compared to other countries, due to availability of antibiotics as over the counter drugs, educational programs ought to be conducted and prescription of antibiotics should be restricted to the findings of laboratory findings. </p>
