fa ویژگی‌های بالینی مجرای شریانی باز در نوزادان نارس و عوامل مادری–نوزادی مرتبط Clinical Characteristics of Patent Ductus Arteriosus in Preterm Neonates and Associated Maternal-Neonatal Factors medicine, paraclinic medicine, paraclinic پژوهشي Research مقدمه مجرای شریانی باز (PDA) یکی از مشکلات شایع در نوزادان نارس است که می‌تواند با عوارض قابل‌توجهی همراه باشد. با وجود مطالعات متعدد درباره ناهنجاری‌های مادرزادی قلب، داده‌های متمرکز و مبتنی بر جمعیت‌های محلی درباره شیوع PDA در نوزادان نارس همچنان محدود است. این مطالعه با هدف تعیین شیوع PDA و بررسی ارتباط آن با سن بارداری، جنس نوزاد و بیماری‌های زمینه‌ای مادر انجام شد. روش کار این پژوهش به‌صورت مقطعی–تحلیلی و با استفاده از داده‌های گذشته‌نگر در مرکز تخصصی اطفال سمنان انجام شد. پرونده ۱۴۹۰ نوزاد نارس بستری طی سال‌های ۱۳۹۸ تا ۱۴۰۱ بررسی شد. تشخیص PDA بر اساس یافته‌های اکوکاردیوگرافی ثبت‌شده در پرونده بیماران صورت گرفت. داده‌ها با استفاده از آزمون کای‌دو در نرم‌افزار SPSS نسخه ۲۶ تحلیل شدند و سطح معناداری 05/0در نظر گرفته شد. یافته‌ها شیوع PDA در نوزادان نارس 11/17٪ بود. ارتباط معناداری بین کاهش سن بارداری و بروز PDA مشاهده شد (p=0.001)، به‌گونه‌ای که 6/70٪ موارد در نوزادان با سن بارداری کمتر از ۳۵ هفته رخ داده بود. همچنین بین جنس نوزاد و وجود PDA ارتباط آماری معنادار یافت شد (p=0.000). بین بیماری‌های زمینه‌ای مادر و بروز PDA ارتباط معناداری مشاهده نشد. نتیجه‌گیری PDA  در نوزادان نارس شیوع قابل‌توجهی دارد و با سن بارداری و جنس نوزاد ارتباط نشان می‌دهد. این یافته‌ها تصویری مبتنی بر داده‌های بومی از الگوی بروز PDA ارائه می‌دهد و می‌تواند مبنایی برای مطالعات تحلیلی گسترده‌تر در آینده باشد.   Background and Objective: Patent ductus arteriosus (PDA) is a common condition among preterm neonates and may be associated with significant complications. Despite numerous studies on congenital heart diseases, focused and population-based data regarding the prevalence of PDA in preterm infants remain limited in local settings. This study aimed to determine the prevalence of PDA and to evaluate its association with gestational age, neonatal sex, and maternal underlying conditions. Methods This retrospective cross-sectional study was conducted at a tertiary pediatric center in Semnan, Iran. Medical records of 1,490 preterm neonates admitted between 2019 and 2022 were reviewed. PDA diagnosis was confirmed based on documented echocardiographic findings. Data were analyzed using the chi-square test in SPSS version 26, and a p-value <0.05 was considered statistically significant. Results The prevalence of PDA among preterm neonates was 11.17%. A significant association was observed between lower gestational age and the occurrence of PDA (p=0.001), with 70.6% of affected neonates born at less than 35 weeks of gestation. A statistically significant relationship was also found between neonatal sex and PDA (p=0.000). No significant association was detected between maternal underlying diseases and PDA. Conclusion PDA has a considerable prevalence among preterm neonates and is associated with gestational age and neonatal sex.
