fa ارزش تشخیصی بیومارکرهای التهابی ESR و CRP در افتراق موارد شدید و متوسط بیماران کووید-19 بستری در بیمارستان شهید بهشتی کاشان در سال 1400 Diagnostic value of inflammatory biomarkers of ESR and CRP in differentiating severe and moderate cases of COVID-19 patients hospitalized in Shahid Beheshti Hospital in Kashan, Iran, in 2021 medicine, paraclinic medicine, paraclinic پژوهشي Research <div style="text-align: justify;">زمینه و هدف: سطوح بیومارکرهای التهابی ESR و CRP به طور قابل توجهی در مراحل اولیه بیماری کووید-19 افزایش می یابد و سطوح آن با شدت بیماری مرتبط است. مطالعه حاضر برای تعیین ارزش تشخیصی CRP و ESR در بیماری کووید-19 انجام شد. روش‌ها: این مطالعه مقطعی روی 287 بیمار مبتلا به کووید-19 بستری در بیمارستان شهید بهشتی کاشان در سال 1400 انجام شد. پرسشنامه حاوی اطلاعات دموگرافیک، علائم بالینی و یافته‌های آزمایشگاهی و پیامد نهایی بیمار با مرور پرونده های پزشکی و سیستم HIS تکمیل گردید. بیماران برحسب شدت درگیری ریه در سی تی اسکن و SPO2 به دو گروه متوسط و شدید تقسیم شدند. داده‌ها وارد نرم‌افزار SPSS-16 شد و میزان ESR و CRP بین دو گروه موردمقایسه قرار گرفت و حساسیت، اختصاصیت، ارزش اخباری مثبت و منفی و نقطه برش تعیین و منحنی ROC ترسیم شد. </div> <div style="text-align: justify;">یافته‌ها: از 287 بیمار، 151 نفر (6/52%) زن و 136 نفر (4/47%) مرد بودند و میانگین سنی بیماران 45/18±14/56 سال بود. فشارخون بالا (5/35%) شایعترین بیماری زمینه‌ای بود. تنگی نفس 8/56% و سرفه 3/52% شایع‌ترین علائم بالینی بودند. 1/34% بیماران به فرم شدید بیماری مبتلا بودند. در نهایت، 18 بیمار (3/6%) فوت کردند. تست ESR با نقطه برش 30 و حساسیت 6/81%، ویژگی 4/71% و تست CRP با نقطه برش 38 و حساسیت 7/84%، ویژگی 5/82% دقت لازم برای تشخیص شدت بیماری کووید-19 را دارند.</div> <div style="text-align: justify;">نتیجه‌گیری: طبق یافته‌های مطالعه حاضر ESR با نقطه برش 30  و تست CRP با نقطه برش 38، دقت تشخیصی لازم برای ارزیابی شدت بیماری کووید-19 را داشتند. لذا در بررسی اولیه بیماران کووید-19 باتوجه به نقطه برش این دو بیومارکر مهم باید موارد شدید بیماری سریعاً شناسایی و اقدامات درمانی لازم را هرچه سریعتر برای پیشگیری از پیامدهای ناگوار آغاز کرد.</div> <div style="text-align: justify;">Background and Aim: The levels of inflammatory biomarkers ESR and CRP are significantly increased in the early stages of the COVID-19 and their levels are related to the severity of the disease. The present study was conducted to determine the diagnostic value of CRP and ESR in COVID-19. Methods: This cross-sectional study was conducted on 287 patients with COVID-19 hospitalized in Shahid Beheshti Hospital in Kashan, Iran, in 2021. A questionnaire containing demographic data, clinical symptoms, laboratory findings and final outcome was completed by reviewing the medical records and HIS system. Patients were divided into moderate and severe groups according to the severity of lung involvement in CT scan and oxygen saturation (SPO2). The data were entered into SPSS-16 software and the ESR and CRP levels were compared between the two groups. Moreover, the sensitivity, specificity, positive and negative predictive values, and the cut-off point were determined and the ROC curve was drawn. Results: Out of 287 patients, 151 (52.6%) were women and 136 (47.4%) were men, and the mean age of the patients was 56.14±18.45 years. Hypertension (35.5%) was the most common underlying disease. Shortness of breath (56.8%) and cough (52.3%) were the most common clinical symptoms. In this study, 34.1% of the patients had a severe form of the disease. Finally, 18 patients (6.3%) died. ESR test with a cut-off point of 30, sensitivity of 81.6%, and specificity of 71.4% and CRP test with a cut-off point of 38, sensitivity of 84.7%, and specificity of 82.5% have the necessary accuracy to diagnose the severity of COVID-19. Conclusion: According to the current findings, ESR with a cut-off point of 30 and CRP with a cut-off point of 38 had the necessary diagnostic accuracy to assess the severity of COVID-19.
