مقایسه میدازولام از طریق بینی و آتروپین خوراکی به عنوان پیش مداوا در بیهوشی اطفال

<p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> با توجه به اهمیت آرامش و عدم اضطراب هنگام ورود اطفال به اتاق عمل و تاثیر شناخته شده آن و نیز مشکلات روش های فعلی تجویز پیش مداواها و به منظور تعیین تاثیر بکارگیری میدازولام در ایران، این تحقیق برای مقایسه میدازولام از طریق بینی و آتروپین خوراکی به عنوان پیش دارو، در بیهوشی اطفال مراجعه کننده به بیمارستان کودکان مفید سال های 76-1375 انجام گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> این مطالعه به روش کارآزمایی بالینی تجربی دوسوکور بر روی 100 کودک که به طور تصادفی به دو گروه تقسیم شدند، صورت پذیرفت. پنجاه بیمار میدازولام از طریق بینی و آتروپین خوراکی نیم ساعت قبل از جدایی از والدین دریافت داشتند و به 50 بیمار دیگر مرفین آتروپین عضلانی نیم ساعت قبل از جدایی از والدین تجویز گردید. آرام بودن و یا وجود اضطراب کودک در 3 مرحله، اول هنگام جدایی از والدین، دوم به هنگام القای بیهوشی، سوم در ریکاوری مورد بررسی قرار گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>یافته ها:</strong> از 100 نفر مورد بررسی، گروه ها به لحاظ سن، جنس، وزن و فشار خون مشابه بوده و در گروه میدازولام، جدایی از والدین بهتر و القای بیهوشی آرام تر و زمان ریکاوری کوتاه تر مشاهده شد به طوری که میزان عدم اضطراب جزئی آن در گروه شاهد 28 و در گروه مورد 76% بود (P<0.001) و در گروه مرفین، میزان بروز تهوع و استفراغ و همچنین ترشحات راه های هوایی بیشتر از گروه میدازولام می باشد و بروز فلاشینگ در گروه میدازولام مختصری بیشتر از گروه مرفین مشهود بود. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> میدازولام از طریق بینی و آتروپین خوراکی به عنوان داروی مناسب برای پیش دارو در بیهوشی اطفال توصیه می گردد. </p>
<p align="justify"><strong>History and Objectives:</strong> Due to the importance of calmness and not having anxiety on entry to operation room in children and its known effect and the present problems with the administration of premedications and in order to define the effect of Midazolam administration in Iran, this study was carried out to compare intranasal Midazolam with oral atropine as premedication in pediatric anesthesia in referrers to Mofid children hospital during 1375 and 1376. </p><p align="justify"><strong>Materials and Methods:</strong> A double blind experimental clinical trial was performed on 100 children whom were classified by random sampling into 2 groups. Intranasal Midazolam and oral atropine were administrated to 50 patients and another 50 patients were premedicated by intramuscular morphine half an hour before separation from their parents. The calmness and/or the anxiety of the child were evaluated in three stages: First, on separation from parents second, at the time of induction of anesthesia and third, at the time of recovery. </p><p align="justify"><strong>Results:</strong> Age, sex, weight and blood pressure of the two groups were similar in these 100 patients. In the Midazolam group, there was a better toleration of separation from their parents, easier anesthesia induction and a shorter recovery time. The prevalence of lack of even slight degrees of anxiety was 28% and 76% in the control and case groups respectively (P<0.001). Nausea, vomiting and airway secretions were more common in the morphine group than the Midazolam one. Flushing was slightly more common in the Midazolam group than the morphine one. </p><p align="justify"><strong>Conclusion:</strong> Intranasal Midazolam and oral Atropine are recommended as appropriate premedication in pediatric anesthesia. </p>
