fa بررسی سطح سرمی IgE در بیماران آسمی و رابطه آن با شدت بیماری در مراجعه کنندگان به بیمارستان های آموزشی دانشگاه علوم پزشکی ایران طی سال 1373 عمومى General پژوهشي Research <p style="DIRECTION: rtl" align="justify"><strong>سابقه و هدف:</strong> با توجه به شیوع و روند رو به افزایش آسم و عوارض شناخته شده آن و تناقض های موجود در مورد میزان IgE با شدت بیماری و به منظور تعیین سطح سرمی IgE و رابطه آن با شدت بیماری آسم، این تحقیق بر روی مراجعه کنندگان به بیمارستان های آموزشی دانشگاه علوم پزشکی ایران طی سال 1373 انجام گرفت. </p><p style="DIRECTION: rtl" align="justify"><strong>مواد و روش ها:</strong> پژوهش حاضر با روش Cross Sectional صورت پذیرفت. تشخیص بیماران آسمی و عملکرد روی بیماران مطابق استاندارد بوده و کسانی را که معتاد به سیگار و مبتلایان به انگل و به بیماری های تنفسی غیر از آسم بودند از مطالعه حذف گردیدند. سطح سرمیطبیعی IgE به روش ELISA از روی نمونه های خون تعیین و مقادیر کمتر از 200 به عنوان طبیعی و شدت بیماری را بر حسب تعداد حملات در سال به صورت خفیف کمتر از 50، متوسط 50 تا 300 و شدید بیشتر از 300 تلقی و ارتباط سطح سرمی با شدت بیماری و علایم آتوپی با استفاده از آمار توصیفی ارایه گردید. </p><p style="DIRECTION: rtl" align="justify"><strong>یافته ها:</strong> طی مدت مورد بررسی، 101 نفر مطالعه شدند که 58% آن ها دارای سطح سرمی بیشتر از طبیعی بودند و بین سطح سرمی IgE با شدت بیماری و نیز بروز کهیر همبستگی دیده شد ولی با سایر علایم آتوپی و علایم سرفه، خلط، تنگی نفس، حملات آسم شبانه ارتباطی مشاهده نگردید. پنجاه درصد بیماران میزان PEFR آن ها کمتر از 60% بود و هرچه طول بیماری بیشتر بود عملکرد ریوی آن ها کمتر و بین سطح سرمی IgE با میزان PEFR آن ها همبستگی وجود داشت. </p><p style="DIRECTION: rtl" align="justify"><strong>نتیجه گیری:</strong> بین سطح سرمی IgE و عملکرد ریوی همبستگی معکوس وجود دارد ولی با علایم بالینی ارتباطی ملاحظه نشد. </p> <p align="justify"><strong>History and Objective:</strong> Due to the increasing prevalence of asthma and its known complication and the existing controversies about the correlation of IgE level and disease severity and in order to define the serum IgE Level and this correlation, this study was performed on referrers to educational hospitals of Iran University of Medical Sciences in 1373. </p><p align="justify"><strong>Materials and Methods:</strong> This study was performed by cross sectional method. The diagnosis of asthma and the management of asthmatic patients were according to standard. Smokers, patients with parasitic infestation and patients affected by pulmonary diseases other than asthma were excluded from the study. Normal serum IgE Level on blood samples was defined by ELISA method. Values less than 200 were considered as normal. Disease severity was classified as mild (Less than 50 episodes per year), moderate (Between 50 to 300 episodes per year) and severe (More than 300 episodes per year) and by using descriptive statistics relationship between serum IgE Level and disease severity and atopic manifestations were described. </p><p align="justify"><strong>Results:</strong> 101 patients were evaluated during the mentioned period. In 58% of the serum IgE Level was abnormally elevated and serum IgE level correlated with disease severity and presence of urticaria. There was no correlation between serum IgE Level from one side and atopic manifestations, cough, sputum, dyspnea and nocturnal asthmatic attacks from the other side. PEFR was less than 50% in 60% of cases. There was an inverse relationship between the length of the disease and the pulmonary function.
