fa ارتباط پلی‌مورفیسم‌‌‌‌‌‌‌‌‌‌‌‌های تک‌نوکلئوتیدی ژن آدیپونکتین با سنّ ابتلا و شروع انسولین‌درمانی در بیماران مبتلا به دیابت نوع دو Association between adiponectin gene single nucleotide polymorphisms with age of diabetes onset and initiating insulin therapy in patients with type 2 diabetes medicine, paraclinic medicine, paraclinic كاربردي Applicable <div style="text-align: justify;">سابقه و هدف: ابتلا به دیابت نوع دو در سنین پایین در حال افزایش است، احتمالاً عوامل ژنتیکی نقش مهمّی در شروع زودرس دیابت نوع دو دارند. هدف از مطالعه حاضر، بررسی تأثیر پلی‌مورفیسم‌های تک‌نوکلئوتیدی ژن آدیپونکتین (T/G 45+)2241766rs، (G/T 276+)1501299 rs و (G/A 11391-)17300539rs بر سنّ ابتلا به دیابت و سنّ شروع انسولین‌درمانی در بیماران مبتلا به دیابت نوع دو بود. مواد و روش‌ها: در این مطالعه موردی - شاهدی، 183 بیمار مبتلا به دیابت نوع دو و 155 فرد سالم شرکت کردند. داده‌های جمعیت‌شناختی و تن‌سنجی جمع‌آوری شدند. بررسی ژنوتایپی پلی‌مورفیسم‌ها با استفاده از روش تقویت سیستم مقاوم به جهش واکنش زنجیره‌ای پلی‌مراز (T-ARMS-PCR) انجام شد. داده‌ها با استفاده از نرم‌افزارهای SPSS نسخه 26 تجزیه‌وتحلیل شدند. نتایج: طبق نتایج، در  T/G45+ SNP سنّ ابتلا به دیابت و سنّ شروع انسولین‌درمانی در حاملین ژنوتایپ TG به‌طور معنی‌داری کمتر از حاملین ژنوتایپ GG بود. (به‌ترتیب 0/035 = P، 0/048 = P). در T/G45+  SNP، 48/6% از بیماران با سنّ ابتلای <35 سال، حامل ژنوتایپ TG بودند (0/013 =P) و ژنوتایپ TG (0/006 =P، 7/994 –1/430 =CI، 2/381 =OR) به‌طور معنی‌داری با افزایش خطر ابتلا به دیابت در سنّ کمتر از 35 سال مرتبط بود. نتیجه‌گیری: طبق نتایج پژوهش حاضر، T/G45 + SNP از ژن آدیپونکتین اثر قابل‌‌توجهی در شروع زودهنگام T2DM و پیشرفت سریع بیماری دارد. بررسی واریانت‌های ژنتیکی مؤثر می‌تواند در غربالگری پیش از ابتلا به دیابت، به‌منظور تشخیص زودهنگام و ارائه راهکارهای پیشگیرانه، مفید باشد.</div> <div style="text-align: justify;">Background: The incidence of type 2 diabetes is growing at an early age. Genetic factors probably play an important role in the early onset of type 2 diabetes. The present study aimed to effect of single nucleotide polymorphisms of adiponectin gene rs2241766 (+45 T/G), rs1501299 (+276 G/T) and rs17300539 (-11391G/A) on the age of diabetes onset and initiating insulin therapy in patients with type 2 diabetes. Materials and Methods: In this case-control study, 183 patients with type 2 diabetes and 155 healthy individuals participated. Demographic and anthropometric data were collected. Genotyping was performed by using amplification refractory mutation system-PCR (ARMS-PCR). Data were analyzed by using SPSS software (version 26). Results: According to results, in rs2241766 (SNP+45 T/G), the age of onset of diabetes and age of starting insulin therapy in the TG genotype carriers was significantly less than in TT genotype carriers (Respectively P=0.035, P=0.048). In SNP+45 T/G, 48.6% of patients with diabetes onset age<35 years were carrying the TG genotype (P=0.013). TG genotype (OR=2.381, 95% CI:1.430–7.994, P=0.006) was associated with the significantly increased risk of diabetes onset age <35 years. Conclusion: According to the results of the present study, SNP+45 T/G of the adiponectin gene has a significant effect on the early onset of T2DM as well as the rapid progression of the disease.
