fa بررسی دلایل تفاوت در شدت عوارض ویروس کرونا در افراد مختلف: یک مطالعه مروری Investigation the reasons for varying the severity of COVID-19 from person to person: A review study medicine, paraclinic medicine, paraclinic مروري Review <div style="text-align: justify;">سابقه و هدف: بیماری COVID-19 در اثر ویروس SARS-COV-2 ایجاد می‌گردد و به‌طور عمده بافت ریه را مورد تهاجم قرار می‌دهد. علاوه‌بر این، بافت‌های دیگر نیز مورد تهاجم ویروس قرار می‌گیرد. گزارش‌ها نشان داده‌اند که تظاهرات بالینی در بین بیماران کووید-19 از حالت بدون علامت تا علائم حادّ تنفسی در ریه و بروز شوک سپتیک با تأثیر بر سیستم ایمنی در ارگان‌های دیگر متغیر است. هدف از این مطالعه مروری، بررسی مکانیسم‌های مولکولی مرتبط با بیماری‌زایی SARS-COV-2 و ارتباط این مکانیسم‌ها با شدت تظاهرات بالینی گوناگون ایجادشده در بیماران بود. مواد و روش ها: مقاله‌های منتشرشده توسط محققان در پایگاه‌های PubMed و Google scholar از سال 2019 تا 2021 براساس کلیدواژه‌های SARS-COV-2، COVID-19 و disease severity جمع‌آوری شد و مورد بررسی قرار گرفت. نتایج: پس از ورود SARS-COV-2  به درون بدن و فعال‌شدن سیستم ایمنی به دلایلی همچون افزایش تولید NET، استرس اکسیداتیو، فعال‌شدن مسیرهای مرگ سلولی شامل فروپتوز و درنهایت انفجار طوفان سایتوکاینی و پایروپتوزیز، شدت این بیماری تشدید می‌شود و مرگ‌ومیر افزایش می‌یابد. علاوه‌بر این، بیان مولکولی NLRP3 می‌تواند نقش عظیمی در ایجاد طوفان سایتوکاینی در بیماران مبتلا  داشته باشد که برخی از افراد بسته به سیستم ایمنی‌شان برای مقابله با ویروس  SARS-COV-2به دلیل تکثیر NLRP3 و افزایش سایر مکانیسم‌های مولکولی مخرب، مراحل مختلف از شدت بیماری را نشان می‌دهند. نتیجه‌گیری: افراد جامعه بسته به واکنش متفاوت سیستم ایمنی‌شان، شدت متفاوتی از بیماری کووید-19 را نشان می‌دهند. به نظر می‌رسد مکانیسم‌های سلولی و مولکولی گوناگون در افراد مبتلا فعال می‌شوند که بیان NLRP3 سهم عظیمی داشته باشد. </div> <div style="text-align: justify;">Background: COVID-19 is caused by the SARS-COV-2 virus and mainly affects the lung tissue. In addition, other tissues are attacked by the virus. Reports have shown that clinical manifestations in Covid-19 patients vary from asymptomatic to acute respiratory symptoms in the lung and septic shock affecting the immune system in other organs. This review article aimed to investigate the molecular mechanisms associated with SARS-COV-2 pathogenicity and the relationship between these mechanisms and the severity of various clinical manifestations in patients. Materials and Methods: Published articles by researchers in PubMed and Google scholar databases from 2019 to 2021 were collected and reviewed based on the keywords SARS-COV-2, COVID-19 and disease severity. Results: After entering the SARS-COV-2 virus into the body and the activation of the immune system for reasons such as increased NET production, oxidative stress, activation of cell death pathways including ferroptosis and finally the explosion of cytokine storm and pyroptosis, exacerbate the severity of the disease and increase mortality. In addition, the molecular expression of NLRP3 can play a major role in the development of cytokine storms in patients, some depend on their immune system to fight the virus due to the proliferation of NLRP3 and other destructive molecular mechanisms, thus showing the different severity of the disease. Conclusion: Individuals in the community show varying degrees of Covid-19 disease depending on the different reactions of their immune systems.
