fa بررسی عوامل مؤثر بر پیش‌آگهی کووید-19 در بیماران مبتلا به بیماری‌های روماتیسمی التهابی: یک مطالعه کوهورت گذشته‌نگر چندمرکزی medicine, paraclinic medicine, paraclinic پژوهشي Research <strong><span style="font-family:B Zar;"><span style="font-size:9.0pt;">سابقه و هدف:</span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> بیماری کووید-19 ناشی از </span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">ویروس </span></span></strong><span dir="LTR"><span style="font-family:Times New Roman,serif;"><span style="font-size:9.0pt;">SARS-CoV-2</span></span></span><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> بوده که برای نخستین‌بار در دسامبر 2019 در ووهان چین گزارش شد. بیماری&shy;های روماتیسمی شامل طیفی از اختلالات اتوایمیون، خودالتهابی مزمن و مولتی‌سیستم بوده، مبتلایان به آن در خطر بیشتر ابتلا به این عفونت قرار دارند. این مطالعه به‌منظور </span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">بررسی عوامل مؤثر بر پیش‌آگهی کووید-19 در بیماران مبتلا به بیماری&shy;های التهابی روماتیسمی انجام گرفت.</span></span></strong><br> <strong><span style="font-family:B Zar;"><span style="font-size:9.0pt;">مواد و روش&shy;ها:</span></span></strong> <strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">این مطالعه برروی 207 بیمار روماتیسمی مبتلا به کووید-19 با </span></span></strong><span dir="LTR"><span style="font-family:Times New Roman,serif;"><span style="font-size:9.0pt;">PCR</span></span></span><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> مثبت که از اردیبهشت 1399 لغایت خردادماه 1400 به سه مرکز درمانی در شهرهای کاشان، تبریز و تهران مراجعه کرده بودند، انجام گرفت. </span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">نمونه‌گیری به‌روش غیراحتمالی و به‌صورت دردسترس انجام شد.</span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> داده&shy;</span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">&shy;</span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">های جمع&shy;آوری‌شده، ازطریق نرم&shy;افزار </span></span></strong><span dir="LTR"><span style="font-family:Times New Roman,serif;"><span style="font-size:9.0pt;">SPSS</span></span></span><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> نسخه 26 و با استفاده از آزمون&shy;&shy;های کای اسکوئر، تی مستقل، تحلیل واریانس و رگرسیون لجستیک مورد تجزیه‌وتحلیل قرار گرفت.</span></span></strong><br> <strong><span style="font-family:B Zar;"><span style="font-size:9.0pt;">نتایج:</span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> یافته&shy;ها نشان داد که با توجه به مقادیر </span></span></strong><span dir="LTR"><span style="font-family:Times New Roman,serif;"><span style="font-size:9.0pt;">OR</span></span></span><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">، شانس پیش&shy;آگهی ضعیف در بین بیماران مرد 48/4 برابر بیماران زن‌؛ در بیمارانی که دارای بیماری ریوی بوده&shy;اند، 55/5 برابر بیمارانی ‌است که بیماری ریوی نداشته&shy;اند؛ و در بین بیمارانی که در زمان ابتلا به کووید-19 در فاز فعال بیماری روماتیسمی بوده&shy;اند، 09/5 برابر بیمارانی ‌است که در فاز غیرفعال بیماری قرار داشته&shy;اند و در بین بیمارانی که یافته&shy;های </span></span></strong><span dir="LTR"><span style="font-family:Times New Roman,serif;"><span style="font-size:9.0pt;">CT</span></span></span><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> آن‌ها دارای شدت متوسط تا شدید بوده، 69/25 برابر بیمارانی ‌است که یافته&shy;های </span></span></strong><span dir="LTR"><span style="font-family:Times New Roman,serif;"><span style="font-size:9.0pt;">CT</span></span></span><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> آن‌ها خفیف و نرمال بوده ‌است.</span></span></strong><br> <strong><span style="color:black;"><span style="font-family:B Zar;"><span style="font-size:9.0pt;">نتیجه&shy;گیری:</span></span></span></strong> <strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">نتایج مطالعه حاضر نشان داد &shy;که پیش&shy;آگهی کووید-19 در بیماران روماتیسمی در مردان‌، افراد دارای بیماری ریوی زمینه&shy;ای، بیماران دچار فاز فعال بیماری روماتیسمی و در آن‌هایی که درگیری متوسط تا شدید در </span></span></strong><span dir="LTR"><span style="font-family:Times New Roman,serif;"><span style="font-size:9.0pt;">CT</span></span></span><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;"> اسکن از ریه داشته&shy;اند، ضعیف بود</span></span></strong><strong><span style="font-family:B Lotus;"><span style="font-size:9.0pt;">.</span></span></strong> Introduction: Covid-19 is caused by the SARS-CoV-2 virus, which was first reported in December 2019 in Wuhan, China. Rheumatic diseases include a range of autoimmune disorders, chronic auto inflammatory, and multisystem disease, and those with the disease are at greater risk for developing the infection. This study was performed to evaluate the factors affecting the prognosis of Covid-19 in patients with rheumatic inflammatory diseases. Materials and Methods: This study was performed on 207 rheumatic patients with Covid-19 with PCR positive who referred to three medical centers in Kashan, Tabriz and Tehran from May 2020 to June 2021. Sampling was done by non-probability method and Convenience. The collected data were analyzed by SPSS software version 26 using Chi-square, independent t-test, analysis of variance and logistic regression. Results: The results showed that according to OR values, the chance of poor prognosis among male patients is 4.48 times higher than female patients, in patients with lung disease is 5.55 times higher than patients without lung disease; and among the patients who were in the active phase of rheumatic disease at the time of Covid-19, it is 5.09 times more than the patients who were in the inactive phase of the disease and among the patients whose CT findings were moderate to severe was 25.69 times higher than patients whose CT findings were mild and normal.
