fa بررسی فراوانی استافیلوکوکوس اورئوس جدا شده از عفونت های پوست و بافت نرم و تعیین الگوی حساسیت آنتی بیوتیکی بیماران مراجعه کننده به بیمارستان پوست رازی تهران طی سال های 94-1393 عمومى General پژوهشي Research <p dir="RTL" style="margin-bottom:0cm;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:9.0pt;">سابقه و هدف:</span></span></strong> <strong><em><span style="font-family:b lotus;"><span style="font-size:9.0pt;">استافیلوکوکوس اورئوس</span></span></em></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;"> رایج &shy;ترین عامل ایجاد عفونت پوست و بافت نرم است. این مطالعه به&shy; منظور تعیین فراوانی <em>اسـتافیلوکوکوس اورئوس</em> جدا شده از عفونت&shy; های پوست و بافت نرم و تعیین الگوی حساسیت آنتی&shy; بیوتیکی در بیماران مراجعه کننده به بیمارستان پوست رازی تهران انجام شد. </span></span></strong></p> <p dir="RTL" style="margin-bottom:0cm;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:9.0pt;">مواد و روش&shy; ها:</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;"> این مطالعه توصیفی-مقطعی روی 400 بیمار دارای عفونت پوست و بافت نرم مراجعه &shy;کن</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">نده به بیمارستان پوست رازی تهران طی سال&shy; های 94-1393 انجام پذیرفت. از سواپ&shy; های استریل برای جمع &shy;آوری نمونه از عفونت&shy; های پوستی استفاده شد. ایزوله &shy;های <em>استافیلوکوکوس اورئوس</em> با</span></span></strong> <strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">استفاده از تست&shy; های بیوشیمیایی</span></span></strong> <strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">نظیر رنگ &shy;آمیزی گرم، کاتالاز، کواگولاز، تست </span></span></strong><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:9.0pt;">DNase</span></span></span> <strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">و تخمیر قند مانیتول تایید گردید. </span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">الگوی حساسیت آنتی&shy; بیوتیکی با روش انتشار دیسک براساس الگوی</span></span></strong><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:9.0pt;">CLSI </span></span></span><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">&nbsp;انجام پذیرفت.</span></span></strong></p> <p dir="RTL" style="margin-bottom:0cm;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:9.0pt;">نتایج:</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;"> ا</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">ز 400 بیمار، 205 نمونه <em>استافیلوکوکوس اورئوس</em> (3/51 درصد) جداسازی و شناسـایی شـد. تعداد </span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">96 ایزوله (8/46 درصد)<em> استافیلوکوکوس اورئوس </em>مقاوم به متی&shy; سیلین (</span></span></strong><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:9.0pt;">MRSA</span></span></span><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">) بودند. هم&shy; چنین،</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;"> 6/94 درصد مقاوم به پنی &shy;سیلین بوده و همه ایزوله &shy;ها به ونکو&shy;مایسین و لینزولید حساس بودند. حساسیت به داپتومایسین (98 درصد) بود و 116 ایزوله (6/56 درصد) مقاوم به چنددارو (</span></span></strong><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:9.0pt;">MDR</span></span></span><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;">) بودند.</span></span></strong></p> <p dir="RTL" style="margin-bottom:0cm;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:9.0pt;">نتیجه گیری:</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:9.0pt;"> بیش از نیمی از عفونت &shy;های پوست و بافت نرم در این مطالعه ناشی از <em>استافیلوکوکوس اورئوس</em> بود و بیش از 48 درصد از ایزوله &shy;ها مقاوم به متی&shy; سیلین بودند. بیشترین مقاومت به پنی&shy; سیلین مشاهده شد.</span></span></strong></p> <p style="text-align: justify;"><span style="font-family:times new roman;"><strong>Background:</strong><em> Staphylococcus aureus</em> <em>(S. aureus)</em> is the most common cause of skin and soft tissue infections. This study aimed to determine the prevalence of <em>S. aureus</em> isolated from skin and soft tissue and antibiotic susceptibility pattern among the patient hospitalized in Razi skin hospital (Tehran-Iran).</span></p> <p style="text-align: justify;"><span style="font-family:times new roman;"><strong>Materials and Methods:</strong> This cross-sectional study was conducted on patients (n=400) with skin and soft tissue infections in Razi skin hospital. Sterilized swabs were used to collect the skin infection samples. <em>S. aureus</em> isolates were confirmed using biochemical tests (gram staining, catalase, coagulase, DNase test and manitol fermentation tests). &nbsp;</span></p> <p style="text-align: justify;"><span style="font-family:times new roman;"><strong>Result:</strong> 51.3 %( 205 out of 400<em>)</em> of isolates were <em>S. aureus.</em> Ninety six (46.8%) of isolates were methicillin and penicillin-resistant <em>S. aureus</em>. All of the isolates showed sensitivity to vancomycin, linezolid. 98% of the isolates were susceptible to daptomycin. One-hundred sixteen (56.6%) isolates were multi- drug resistant.</span></p> <p style="text-align: justify;"><span style="font-family:times new roman;"><strong>Conclusion:</strong> More than half of the skin and soft tissue infections were caused by <em>S.aureus.</em> More than 46 percent of the isolates were methicillin resistant.
