fa بررسی پلی مورفیسم rs7526084 در ناحیه 3َ پایین دست ژن سسترین 2 به عنوان یک مارکر گویا در مطالعه مولکولی بیماری سنگ کلیه Analysis of rs7526084 polymorphism in 3′ downstream of the sestrin 2 gene as an informative marker for molecular diagnosis of kidney stone disease medicine, paraclinic medicine, paraclinic پژوهشي Research <div style="text-align: justify;">سابقه و هدف: بیماری سنگ کلیه یکی از شایع ترین اختلالات مجاری ادراری است که با هتروژنیتی بالینی و ژنتیکی بالا در جمعیت بروز می نماید. سسترین 2 در مسیرهای حیاتی گستردهای، به خصوص استرس اکسیداتیو و ژنوتوکسیک، درگیر می باشد. باتوجه به اهمیت تنظیم نامناسب مسیر استرس اکسیداتیو در بیماری های سیستمیک مانند ناهنجاری های کلیوی، این مطالعه با هدف بررسی ارتباط پلیمورفیسم rs7526084 در ناحیه 3َ پایین دست ژن سسترین 2 و ریسک بروز سنگ کلیه در بیماران جنوب ایران انجام شد. مواد و روش ها: در این مطالعه مورد-شاهدی، 150 فرد مبتلا به سنگ کلیه و 180 فرد سالم که از نظر سن و جنس همسان سازی شده بودند، در فاصله زمانی فروردین تا دی ماه 1394 شرکت داده شدند. تعیین ژنوتیپ پلیمورفیسم rs7526084 با استفاده از روش T-ARMS PCR صورت گرفت. نتایج: <cite>فراوانی آلل </cite><cite>G</cite><cite> در افراد گروه کنترل بالاتر از افراد گروه بیمار بود و درحضور این آلل ریسک بروز بیماری کاهش نشان داد (01/0</cite><cite>=</cite><cite>P</cite><cite>، 91/0-48/0:</cite><cite>CI</cite><cite> 95 درصد، 66/0:</cite><cite>OR</cite><cite>). هم چنین، در حضور ژنوتیپ های هتروزیگوت </cite><cite>CG</cite><cite> (004/0</cite><cite>=</cite><cite>P</cite><cite>، 80/0-30/0:</cite> <cite>CI</cite><cite>95 درصد، 49/0:</cite><cite>OR</cite><cite>) و هموزیگوت </cite><cite>GG</cite><cite> (04/0</cite><cite>=</cite><cite>P</cite><cite>، 96/0-23/0:</cite><cite>CI</cite><cite> 95 درصد، 47/0:</cite><cite>OR</cite><cite>) کاهش خطر بروز سنگ کلیه دیده شد. تحت مدل ژنتیک غالب برای آلل </cite><cite>G</cite><cite> (</cite><cite>GG+GC</cite><cite> در مقابل </cite><cite>CC</cite><cite>)، این آلل خطر بروز بیماری را به طور قابل توجهی کاهش داد (002/0</cite><cite>=</cite><cite>P</cite><cite>، 78/0-31/0:</cite><cite>CI</cite><cite> 95 درصد، 49/0:</cite><cite>OR</cite><cite>). </cite> نتیجه گیری: کاهش خطر بروز سنگ کلیه در حضور آلل G از پلیمورفیسم ژن سسترین 2 احتمالا مدرکی دال بر دخالت مسیر استرس اکسیداتیو در پاتوژنز سنگ کلیه می باشد. </div> <div style="text-align: justify;">Background: Kidney stone disease is one of the most common disorders of urinary tract, manifesting with high clinical and genetic heterogeneity in population. Sestrin 2 is involved in many essential processes, especially oxidative and genotoxic stress. Regarding the importance of oxidative stress pathway deregulation in systemic diseases such as kidney disorders, this study was conducted to investigate the association between rs7526084 3′ downstream polymorphism of the sestrin 2 gene and risk of kidney stone in south Iranian patients. Materials and Methods: In this case-control study, 150 patients with kidney stone disease and 180 age- and gender-matched healthy individuals were participated from March to December 2015. Genotyping of the rs7526084 polymorphism was performed using the T-ARMS PCR method. Results: Frequency of the G allele was higher in controls than in cases, and a reduced risk of the disease was shown in the presence of this allele (OR: 0.66, 95%CI: 0.48-0.91, P=0.01). Also, the risk of the disease was reduced in the presence of heterozygote CG (OR: 0.49, 95%CI: 0.30-0.80, P=0.004) and homozygote GG (OR: 0.47, 95%CI: 0.23-0.96, P=0.04) genotypes. Under the dominant genetic model for the G allele (GG+GC vs. CC), this allele significantly reduced the risk of the disease (OR: 0.49, 95%CI: 0.31- 0.78, P= 0.002).
