مقایسه آنتی بیوتیک پروفیلاکسی تک دوز و چند دوز در پیشگیری از عفونت زخم به دنبال آپاندکتومی بدون عارضه

<p align="justify"><strong>سابقه وهدف:</strong> عفونت زخم به دنبال اعمال جراحی یکی از مهمترین علت های مرگ و میر و ایجاد ناتوانی است. علیرغم پیدایش آنتی بیوتیکها در اواسط قرن بیستم, استفاده گسترده از این داروها باعث پیدایش سوشهای مقاوم و پیچیده تر شدن اداره عفونت زخم جراحی شد. بنابراین تعیین و جوب تجویز آنتی بیوتیک بسیار حائز اهمیت است. در این مطالعه میزان عفونت زخم در دو رژیم آنتی بیوتیک تک دوز و چند دوز در بیمارانی که تحت عمل جراحی آپاندکتومی بدون عارضه قرار گرفته اند مقایسه شد. </p><p align="justify"><strong>مواد و روشها:</strong> این مطالعه نوعی کارآزمایی بالینی تصادفی شده است که بر 118 بیماری که تحت عمل جراحی آپاندکتومی بدون عارضه قرار گرفتند انجام شد. آپاندیسیت بر اساس شرح حال و معاینه بالینی و داده های آزمایشگاهی تشخیص داده می شد. هر دو گروه قبل از عمل یک دوز آنتی بیوتیک شامل مترونیدازول mg/kg 10 و جنتامایسین mg/kg 5/1 به صورت وریدی دریافت می کردند. گروه اول شامل 58 نفر بود که بعد از عمل آنتی بیوتیک دریافت نمی کردند. گروه دوم شامل 60 نفر بود که مدت 48 ساعت پس از جراحی آنتی بیوتیک دریافت می کردن. بیماران به مدت 30 روز از نظر عفونت زخم پیگری شدند. یافتهها براساس آزمون دقیق فیشر تحلیل شد. </p><p align="justify"><strong>یافتهها:</strong> در گـــــــروه اول 2 نفر ( 5/3درصد) و در گروه دوم هم 2نفر (3/3درصد) دچار عفونت زخم شدند ( N.S ). </p><p align="justify"><strong>نتیجهگیری:</strong> انجام تحقق مشابه روی بیماران با اعمال جراحی دیگر توصیه می گردد. تفاوت معنی د اری بین دو رژیم آنتیبیوتیکی در پیشگیری از عفونت زخم وجود ندارد. </p>
<p align="justify"><strong>Background</strong>: Post operative wound infections are among the most common causes of mortality and morbidity. Despite the introduction of new antibiotics in mid 20<sup>th</sup> century their widespread usage was resulted in resistant pathogens that grow and complicate wound infection management. In the present study, we have compared single- and multiple- dose prophylactic antibiotic in non-complicated appendicitis wound infection.</p><p align="justify"><strong>Materials and methods</strong>: 118 non-complicated cases of appendicitis had been enrolled for this randomized clinical trial. Their diagnosis was made according to their physical examination and laboratory findings. Patients were assigned in two groups, both of which received IV metronidazol (10mg/kg) and gentamicin (1.5mg/kg), preoperatively. For 58 patients postoperative antibiotic was discontinued (group A), however, 60 patients have continued their antibiotic regimen for 48 hours (group B). Patients were followed for 30 days.</p><p align="justify"><strong>Results</strong>: Wound infection was reported in 3.45% and 3.33% of the patients in the first and second group, respectively (NS).</p><p align="justify"><strong>Conclusion</strong>: Results have revealed no significant difference between the two regimens.</p>
