گزارش یک مورد دیسفاژی و ضعف عضلات گردنی به همراه بولبارپالسی به عنوان تظاهر اولیه لوپوس

<b>سابقه و هدف:</b> علائم نورو سایکیاتریک از عوارض شناخته شده لوپوس است. هم چنین، درگیری سیستم اعصاب محیطی و مرکزی در لوپوس گزارش شده اند.
<br><b>معرفی بیمار:</b> بیمار خانم متاهل 32 ساله ای بود که از 15 روز قبل بستری با تاریخچه دیسفاژی پیشرونده، تون صدای تودماغی و ضعف عضلات گردن مراجعه کرده بود. وی تاریخچه هیچ بیماری دیگری را بیان نمیکرد. در معاینه نورولوژیک به طور مشخص بولبارپالسی با درگیری اعصاب 11، 10، 9 داشت. و ضعف در نگهداری عضلات گردن مشهود بود. سایر معاینات نیز نرمال بودند. کلیه تست های آزمایشگاهی هماتولوژی، عملکرد کلیه و کبد نرمال بود و در عکس گرافی ریه نکته پاتولوژیکی وجود نداشت. مطالعات EMG و NCV نرمال به دست آمد. جهت نامبرده تست لوپوس آنتی کواگولان، Anti-dsDNA و ANA درخواست شد که در 2 نوبت متوالی مثبت گزارش شد. در MRI مغز ضایعه انفارکت منطبق بر همی سفر مخچه در سمت راست و انفارکت کوچک دیگر در ناحیه پشتی ساقه مغز گزارش شد. تشخیص واسکولیت ثانویه به شبه لوپوس جهت بیمار گذارده شد. بیمار IV-IG با دوز روزانه mg/kg 0/4 به مدت 5 روز همراه با 0/5 گرم متیل پردنیزولون روزانه به مدت 5 روز دریافت نمود و بعد از بهبودی ترخیص گردید. ایشان جهت پیگیری منظم بعدی به متخصص روماتولوژی ارجاع داده شد.
<br><b>نتیجه گیری: </b>لوپوس باید در هر بیماری که با مجموعه علائم و نشانه های مبهم نورولوژیک مراجعه می کند، در نظر گرفته شود.
<b>Background: </b>Neuropsychiatric disorders are among the known complications of systemic lupus erythematosus (SLE). The involvement of both peripheral and central nervous systems has also been reported in SLE.<br><b>Case Report:</b> A 32 year-old woman presented with 15 days history of progressive dysphagia, the nasal intonation of voice with no significant previous medical illness. The neurologic examination showed a marked bulbar (involvement of 9, 10, 11 cranial nerves) and cervical palsy. Other systemic examinations were normal. Hematologic tests, ESR, CRP, renal and hepatic tests and a chest x-ray PA were normal. Moreover, the CSF analysis and electrophysiological studies (EMG, NCV) were normal. The ANA and Anti-ds DNA tests were positive for two consecutive times. Brain MRI revealed infarct lesions in RT cerebellar hemisphere and dorsal brainstem. A diagnosis of vasculitis secondary to liked-lupus was made based on laboratory tests and imaging. Then intravenous immunoglobulins (0.4 g/kg/day) and 0.5 g methyl prednisolone for 5 days were administered. She was discharged for the rheumatologic follow-up. <br>
<b>Conclusion:</b> It can be concluded that SLE should be considered in any patient with obscure neurological problems.
