بررسی فنوتیپی آنزیم های بتالاکتاماز و متالوبتالاکتاماز سودوموناس آئروژینوزای جدا شده از زخم سوختگی در بیمارستان سوانح سوختگی یزد طی سال1391

<p><strong>سابقه و هدف:</strong> <i>سودوموناس آئروژینوزا </i>یک باکتری گرم منفی غیرتخمیری و فرصت طلب است که عامل10 تا 15 درصد عفونتهای بیمارستانی و زخم سوختگی در جهان است. این باکتری با مکانیسمهای مختلف به انواع آنتیبیوتیکها مقاوم میشود. هدف از مطالعه حاضر بررسی فراوانی آنزیمهای بتالاکتاماز با طیف گسترشیافته (ESBL) و آنزیمهای متالوبتالاکتاماز (MBL) در سویههای <i>سودوموناس آئروژینوزای</i> جدا شده از زخم سوختگی است. </p><p><strong>مواد و روشها:</strong> در این مطالعه توصیفی-مقطعی تعداد 180 نمونه زخم سوختگی از بیماران بستری در بیمارستان سوانح سوختگی اخذ شده و کشت داده شد. کلونیهای مشکوک به <i>سودوموناس آئروژینوز </i>ا با روشهای بیوشیمیایی معمول تعیین هویت شدند. برای سنجش حساسیت آنتیبیوتیکی از روش کربی بائر ، آنزیمهای بتالاکتاماز وسیع الطیف از روش Combination Disk و آنزیمهای متالوبتالاکتاماز از روش Etest MBL ا ستفاده شد. </p><p><strong>نتایج:</strong> از 180 نمونه کشت داده شده، 54 (30 درصد) ایزوله بهعنوان <i>سودوموناس آئروژینوزا </i>تعیین هویت شد که 22 درصد (12 نمونه) دارای ESBL و 5/29 درصد (16 نمونه) دارای MBL بودند. در مجموع 42 (79 درصد)،40 (74 درصد)، 40 (74 درصد)، 38 (70 درصد)، 35 (66 درصد)، و 34 ایزوله (62 درصد) بهترتیب نسبت به سفتیزوکسیم، ایمیپنم، جنتامایسین، پیپراسیلین، سفپیم، مروپنم، و ارتاپنم مقاوم بودند. </p><p><strong>نتیجهگیری:</strong> شیوع آنزیمهای ESBL و MBL و مقاومت آنتیبیوتیکی در بیمارستان سوانح سوختگی بالا است و نیاز است که اقداماتی مانند سنجش حساسیت آنتیبیوتیکی، تجویز منطقی آنتیبیوتیکها و کنترل عوامل مساعد کننده انجام شود. با توجه به اینکه آنزیمهای ESBL و MBL کلاسهای گوناگونی دارند و شیوع آنها در مناطق مختلف متفاوت است، لازم است بررسیهای ملکولی در این زمینه انجام گردد. </p>
<p><strong>Background:</strong> Pseudomonas aeruginosa is a gram-negative, non-fermentative bacillus and one of the most common opportunistic human pathogen causing 10-15% of nosocomial and burn wound infections worldwide. These bacteria are resistant to most of the antibiotics. This study aimed to investigate the frequency of extended spectrum β-lactamase (ESBL) and metallo-β-lactamase (MBL) enzymes in P.aeruginosa strains isolated from burn wounds. </p><p><strong>Materials and Methods:</strong> In this descriptive cross-sectional study, 180 wound specimens were collected from patients hospitalized in burn hospital and then cultured. The suspected colonies were identified using the conventional biochemical methods. Susceptibility testing was performed using the Kirby-Bauer method. Moreover, the Combination Disk and E.test methods were used for the determination of ESBLs and MBL, respectively. </p><p><strong>Results:</strong> Fifty-four (30%) out of 180 cultured samples were identified as p.aeruginosa 12 (22%) and 16 (29.5%) isolates were ESBLs and MBL producer, respectively. Overall, 42 (79%), 40 (74%), 40 (74%), 38 (70%), 35 (66%) and 34 (62%) isolates were resistant to ceftizoxime, imipenem, gentamycin, piperacillin, cefepime, meropenem and ertapenem, respectively. </p><p><strong>Conclusion:</strong> Results reveal that the antimicrobial resistance and prevalence of ESBLs and MBL producing P.aeruginosa strains is increasing in our hospital and it is necessary to perform susceptibility testing, control risk factors and reasonably prescribe the appropriate antibiotics. Considering the diversity of ESBL and MBL classes and their prevalence in different areas, performing molecular research is needed. </p>
