fa یافته های کلینیکی و رادیوگرافی آسپیراسیون جسم خارجی در کودکان بستری در بیمارستان هفده شهریور رشت طی سال های 1375 تا 1387 medicine, paraclinic medicine, paraclinic پژوهشي Research <p><strong>سابقه و هدف:</strong> آسپیراسیون جسم خارجی (Foreign body aspiration) در کودکان و به­ویژه در بچه­های 1 تا 3 سال شایع بوده و یک فوریت تهدیدکننده حیات است. این عارضه علت مرگ در 7 درصد کودکان زیر 4 سال می ­ باشد. هدف از این پژوهش تعیین فراوانی نسبی یافته­های شرح‌حال، معاینه فیزیکی و عکس قفسه سینه در کودکان دچار آسپیراسیون جسم خارجی مجاری تنفسی بستری در بیمارستان 17 شهریور رشت می­باشد. </p><p><strong>مواد و روش­ها:</strong> از پرونده ­ های تمامی کودکان بستری شده در بیمارستان 17 شهریور رشت از سال 1375 تا 1378 با تشخیص نهایی آسپیراسیون جسم خارجی، داده­های مورد نیاز جمع­آوری گردید. </p><p><strong>نتایج:</strong> 5/51 درصد کودکان مورد مطالعه 1 تا 3 ساله بوده و شایع­ترین شکایت اصلی بیماران سرفه (34 درصد) و اشکال در تنفس (34 درصد) بود. در تاریخچه بیماری شک اولیه به آسپیراسیون جسم خارجی (78 درصد)، دیسترس تنفسی (74 درصد) و خفگی حین تغذیه (63 درصد) ب ود. شایع­ترین یافته­های معاینه فیزیکی، کاهش صدای یک­طرفه (56 درصد) و رال (45 درصد) بود. شایع­ترین یافته در عکس قفسه سینه، کدورت (34 درصد) و پرهوایی یک­طرفه (32 درصد) بود. شایع­ترین جسم خارجی یافته شده، تخم آفتابگردان (5/34 درصد) بود و نسبت قرارداشتن اجسام خارجی در برونش راست و چپ در کودکان زیر یک­سال (40 درصد) برابر و در سنین دیگر در برونش راست بیشتر بود. </p><p><strong>نتیجه­گیری:</strong> بر اساس مطالعه حاضر، شرح حال و به­ویژه شک اولیه به آسپیراسیون جسم خارجی و خفگی حین تغذیه با توجه به شیوع بالای آنها می‌توانند در تشخیص کمک‌کننده باشد . </p> <p><strong>Background:</strong> Foreign body aspiration (FBA), as a life-threatening emergency, is a common problem in children especially in 1-3 year old age group. FBA is one of the leading causes of death in the 7% of children under 4 years. The purpose of this study was to evaluate the clinical and radiographic findings of foreign body aspiration in children admitted to 17- Shahrivar hospital in Rash.</p><p><strong>Materials and Methods:</strong> In this study, the required data were collected from the medical records of all children with FBA hospitalized in 17- Shahrivar hospital (Rasht, Iran).</p><p><strong>Results:</strong> Fifty-one percent of the children were 1-3 years and the most common complaints of patients were cough (34%) and respiratory distress (34%). In patients' history records, initial suspicion to aspiration was 78%, respiratory distress, 65% and choking, 63%. Common physical examination findings were unilateral decreased pulmonary sound (65%) and rales (45%). The most common findings in CXR were consolidation (34%) and unilateral air trapping (32%).The most frequent foreign body was sunflower seed (34.5%).
