Hydatid cyst drainage with ultrasonography through skin

History and Objectives: In light of prevalence of hydatid cyst, ineffective therapy and difficultly in surgical procedures for all patients and recent successful drainage of by ultrasonography from skin, the present investigation was conducted in patients suffering from hepatic hydatid cyst referred to the Fayze hospital of Isfahan in 1995-97.

Materials and Methods: A semi-experimental study on 8 hydatid cyst and five patients aged 18-44 years was performed. Diagnosis was based on ultrasonographic evidence and IFAT test. Operation was carried out by ultrasonography, type Aloaka and Shiba needle number 19. After injection of the needle into the cyst fluid, the fluid content was drained and 50% of its volume was replaced with alcohol or hypertonic salt as a scolecidal agent for 30 minutes and then aspirated. Size and shape of cyst was recorded before the drainage. Albendozole (400mg) was given to all patients for 10 day before and 20 days after drainage. Patients were followed from 6 months to 26 months (Mean 17 months) and IFAT test, ultrasonography examination and size of cyst were recorded. Changes in the size of cyst were analyzed with paired T-test and the results were analyzed with Mc Nemar test.

Results: Based on IFAT test and ultrasonographic pictures and size of cyst the procedure was successful (P<0.05). Recurrence and anaphylactic response were not observed. No toxic response to alcohol was recorded.

Conclusion: Considering the result of the present study and outcome of similar studies which has been carried out in other countries, drainage of hydatid cyst through skin with ultrasonography guidance for patients suffering from hydatid cyst especially those with cardiac or respiratory complication and those at older age is recommended.
