تأثیر تمرینات ثبات مرکزی با و بدون استفاده از تکنیک الکساندر بر ترس از حرکت، اضطراب و افسردگی در زنان مبتلا به کمردرد مزمن غیراختصاصی

زمینه و هدف: ترس از فعالیت بدنی و عوامل روان‌شناختی می‌توانند در تداوم و تشدید کمردرد مزمن غیراختصاصی نقش داشته باشند. هدف از این مطالعه، بررسی تأثیر تمرینات ثبات مرکزی همراه با تکنیک ارگونومیک الکساندر بر ترس از حرکت، اضطراب و افسردگی در زنان مبتلا به کمردرد مزمن غیراختصاصی بود.
روش‌ها: این کارآزمایی بالینی کنترل ‌شده سه‌گروهی بر روی 63 زن مبتلا به کمردرد مزمن غیراختصاصی انجام شد. شرکت‌کنندگان به‌صورت تصادفی در سه گروه تمرینات ثبات مرکزی همراه با تکنیک ارگونومیک الکساندر (21 نفر)، تمرینات ثبات مرکزی (21 نفر) و کنترل (21 نفر) تقسیم شدند. گروه‌های مداخله به مدت 8 هفته، سه جلسه در هفته و هر جلسه 60 دقیقه تحت درمان توانبخشی قرار گرفتند. ارزیابی ترس از حرکت با پرسشنامه 17 سؤالی تمپا و اضطراب و افسردگی با مقیاس 14 سؤالی بیمارستانی اضطراب و افسردگی (HADS) ارزیابی شدند. برای مقایسه سه گروه از تحلیل واریانس با اندازه‌گیری‌های تکراری استفاده شد.
یافته‌ها: نتایج نشان داد در اثر متقابل )زمان×گروه( در شاخص‌های ترس از حرکت
(F(2,60) = 67.55, P<0.001, partial η²=0.692)، اضطراب (F(2,60)=45.02, P<0.001, partial η²=0.600) و افسردگی (F(2,60) = 131.40, P<0.001, partial η²=0.814) تفاوت معنی داری بین سه گروه مشاهده شد. آزمون تعقیبی توکی نشان داد گروه تمرینات ترکیبی در مقایسه با گروه ثبات مرکزی و کنترل، کاهش معنی‌داری در هر سه شاخص داشت (05/0P<).
نتیجه‌گیری: ترکیب تمرینات ثبات مرکزی با تکنیک ارگونومیک الکساندر، در مقایسه با تمرینات ثبات مرکزی به‌تنهایی، تأثیر بیشتری در کاهش ترس از حرکت، اضطراب و افسردگی زنان مبتلا به کمردرد مزمن غیراختصاصی دارد. افزودن این رویکرد به برنامه‌های توانبخشی می‌تواند مورد توجه فیزیوتراپیست‌ها و متخصصان علوم ورزشی قرار گیرد.
