The Effectiveness of Mindfulness-Based Cognitive Therapy on Depression and Sleep Quality in Patients Undergoing Hemodialysis

Background and Aim: Chronic kidney disease and the hemodialysis process, as a long-term medical condition, impose substantial physical and psychological burdens, predisposing patients to depression and sleep disturbances. The present study aimed to evaluate the effectiveness of mindfulness-based cognitive therapy (MBCT) on depression levels and sleep quality in patients undergoing hemodialysis.
Methods: This quasi-experimental study employed a pretest–posttest design with a control group. The study population consisted of all hemodialysis patients at Shafa Hospital in Kerman, Iran, in 2024. A total of 30 patients (15 in the experimental group and 15 in the control group) were selected through convenience sampling. Data were collected using the Beck Depression Inventory (1961) and the Pittsburgh Sleep Quality Index (1989). The MBCT intervention was conducted over 8 sessions, each lasting 60 minutes, delivered twice weekly for the experimental group, while the control group received no intervention. Data were analyzed using descriptive statistics and analysis of covariance (ANCOVA) in SPSS version 24, with a significance level set at P < 0.05.
Results: The findings indicated that MBCT significantly reduced depression (F = 4.868, P = 0.013, η² = 0.81) and significantly improved sleep quality (F = 5.771, P = 0.043, η² = 0.43) in the experimental group compared with the control group. No significant changes were observed in any of the variables within the control group.
Conclusion: The results suggest that mindfulness-based cognitive therapy may serve as an effective psychological intervention for reducing depression and improving sleep quality in patients undergoing hemodialysis.
