Lipids are essential components of all brain cell membranes. Gangliosides are glycosphingolipids that are widely expressed throughout the body but are particularly abundant in the central nervous system (CNS). They are key constituents of CNS cell membranes and play a vital role in maintaining normal brain function. Monosialotetrahexosylganglioside 1 (GM1) has an important role in the potential neuroprotective mechanisms of cerebral ischemia. A complex homeostatic balance of gangliosides is required for normal CNS function and is therefore tightly regulated through intricate intracellular trafficking and metabolic pathways. Despite promising preclinical studies, there is currently no drug that results in sustained clinical improvement in patients with ischemic stroke. It is well established that developing new therapeutic strategies, including the identification of multitarget drugs capable of modulating diverse biological pathways, is possible by evaluating the various molecular mechanisms involved in neuronal death and tissue damage. Experimental evidence strongly highlights the role of gangliosides in ischemic disease and their potential therapeutic value. In this narrative review, we will discuss the functions of gangliosides that are vital for brain health, examine the alterations in their levels and expression profiles occurring in major neurological disorders, and address their contribution to cellular dysfunction and disease pathogenesis.
