همخوانی بین بیش‌برانگیختگی زیستی و روان شناختی در پاسخ به رفتاردرمانی شناختی (با/ بدون تکنیک‌های فراشناختی) در بیماران مبتلا به اختلال بی‌خوابی مزمن: طرح تک‌موردی با خطوط پایه‌ی چندگانه

سابقه و هدف: بیش‌برانگیختگی یکی از مکانیسم‌های آسیب‌شناختی اصلی در اختلال بی‌خوابی مزمن است. هدف از پژوهش حاضر، مقایسه‌ی همخوانی بین بیش‌برانگیختگی زیستی و روان­شناختی در پاسخ به رفتاردرمانی شناختی و رفتاردرمانی شناختی همراه با تکنیک‌های فراشناختی است.
مواد و روش­ ها: پس از ارزیابی‌های تشخیصی و بررسی ملاک‌های ورود و خروج، چهار بیمار (شامل یک مرد و سه زن؛ طیف سنی: 21 تا 34) در دو گروه رفتاردرمانی شناختی و رفتاردرمانی شناختی به همراه تکنیک‌های فراشناختی قرار گرفتند. تمامی بیماران در شش دفعه، با مقیاس برانگیختگی پیش از خواب و شاخص بازدهی خواب سنجیده شدند. برای تجزیه و تحلیل داده‌ها از معناداری بالینی (درصد بهبودی)، اندازه‌ی اثر و بهبودی تشخیصی استفاده شد.
نتایج: بین بیش‌برانگیختگی زیستی و روان­شناختی در جلسات اوّلیه‌ی درمان، هم­خوانی خیلی کمی دیده می‌شود. همخوانی بین این شاخص‌ها، همان ­طور که جلسات پیش می‌رود، بیشتر می‌شود. همچنین، هر دو درمان توانستند بیش‌برانگیختگی زیستی و روان­شناختی در مراحل پس از درمان و پیگیری 1 و 3 ماهه را، به ­طور معنادار کاهش دهند. اندازه‌ ای اثر به ­دست­ آمده، بین 0/74 تا 1/93 بود. به علاوه، بازدهی خواب تمامی بیماران طی مراحل پس از درمان بین 89 تا 94 درصد و در مرحله‌ی پیگیری 3 ماهه، بین 88 تا 90 درصد بود.
نتیجه‌گیری: به­ عکس جلسات ابتدایی درمان، هم‌خوانی بین بیش‌برانگیختگی زیستی و روان­شناختی، در پایان درمان و پیگیری 3 ماهه افزایش یافت. هر دو درمان می‌توانند به ­طور معناداری بیش‌برانگیختگی را کاهش و بازدهی خواب را افزایش دهند.
