The necessity of establishing a national ovarian biobank network and centralized support mechanisms to safeguard the fertility of cancer patients: A policy brief

Substantial progress in cancer treatment has dramatically improved survival rates, yet gonadotoxic therapies often induce premature ovarian insufficiency (POI), compromising reproductive potential and quality of life for female survivors. Ovarian tissue cryopreservation (OTC) remains the only established fertility preservation option for prepubertal girls and patients requiring urgent treatment initiation. In Iran, systemic barriers limit equitable access to this essential service.
Policy Options: the three key policy options include adopting and promulgating the “National Protocol on Ethics and Reproductive Rights in Cancer,” increasing knowledge, quality, and public awareness, and integrating fertility preservation services into the “National Cancer Care Network.
Recommendation: Establishing a national network of public hospital-based ovarian tissue biobanks supported by centralized coordination and governance mechanisms. This integrated approach incorporates elements of all three policy options, with primary emphasis on option 3 to ensure sustainable implementation, systematic referral pathways, and improved access for cancer patients.
