fa پیش بینی سندرم زجر تنفسی نوزادان (RDS) با استفاده از یادگیری ماشین Predicting neonatal respiratory distress syndrome (RDS) using machine learning عمومى General پژوهشي Research سندرم زجر تنفسی نوزادان (RDS) یکی از شایع‌ترین علل مرگ‌ومیر در نوزادان نارس است که عمدتاً ناشی از کمبود سورفکتانت ریوی می‌باشد. تشخیص به‌موقع این بیماری نقش حیاتی در کاهش مرگ‌ومیر و بهبود پیامدهای بالینی ایفا می‌کند. روش‌های سنتی تشخیص مانند معاینه بالینی و رادیوگرافی قفسه سینه با محدودیت‌هایی از جمله تأخیر در نتیجه‌گیری و دقت نسبی مواجه هستند روش کار: این مطالعه با هدف توسعه یک مدل پیش‌بینی مبتنی بر یادگیری ماشین برای شناسایی زودهنگام سندرم زجر تنفسی نوزادان انجام شد. در این مطالعه گذشته نگر، داده‌های ۳۰۰ نوزاد بستری در بخش مراقبت‌های ویژه نوزادان بیمارستان شهید بهشتی کاشان طی سال‌های ۱۴۰۲ تا ۱۴۰۳ جمع‌آوری و داده‌های بالینی از ۲۳ متغیر استخراج شد؛ با این حال، متغیرهای «نیاز به حمایت تنفسی» و «تجویز سورفکتانت» به دلیل ماهیت درمانی و احتمال ایجاد نشت اطلاعات، از ویژگی‌های ورودی مدل‌های پیش‌بینی حذف شدند. پس از پیش‌پردازش داده‌ها، چهار الگوریتم یادگیری ماشین شامل درخت تصمیم، ID3، k-نزدیک‌ترین همسایه و ماشین بردار پشتیبان در محیط نرم‌افزار RapidMiner پیاده‌سازی شدند. ارزیابی مدل‌ها با استفاده از روش تقسیم داده به دو بخش آموزش (70 درصد) و آزمون (30 درصد) انجام شد و عملکرد آن‌ها با شاخص‌های متداول شامل دقت، حساسیت، ویژگی، دقت مثبت و امتیاز F1 سنجیده شد. یافته ها: نتایج نشان داد الگوریتم درخت تصمیم با دقت کلی ۹۰٪، حساسیت 89٫23٪، ویژگی ۹۲٪، دقت مثبت 96٫67٪ و امتیاز F1 برابر با ۹۲٫۸۰٪ بهترین عملکرد را داشت. همچنین، مساحت زیر منحنی ROC (AUC) برای مدل درخت تصمیم برابر با 0.868 بود که نشان‌دهنده توان تفکیک مناسب مدل است. نتیجه‌گیری: یافته‌ها نشان می‌دهد مدل درخت تصمیم با استفاده از متغیرهای بالینی غیرمرتبط با اقدامات درمانی، از قابلیت مناسبی برای طبقه‌بندی خطر RDS برخوردار است؛ با این حال، پیش از کاربرد بالینی، اعتبارسنجی خارجی و چندمرکزی مدل ضروری است.   Introduction: Neonatal respiratory distress syndrome (RDS) is one of the most common causes of mortality in preterm infants and is primarily associated with pulmonary surfactant deficiency.  Risk stratification of RDS is important for improving clinical outcomes and reducing mortality. Conventional diagnostic approaches, including clinical examination and chest radiography, may have limitations in the early assessment of affected neonates. Method: This study aimed to develop a machine learning-based model for  risk stratification of neonatal respiratory distress syndrome. In this retrospective study, data were collected from 300 infants admitted to the neonatal intensive care unit of Shahid Beheshti Hospital in Kashan between 2023 and 2024. Data were initially extracted for 23 clinical variables. However, respiratory support and surfactant administration were excluded from the predictor set because of their treatment-related nature and potential risk of information leakage, leaving 21 variables for model development. After data preprocessing, four machine learning algorithms, including Decision Tree, ID3, k-nearest neighbors (KNN), and support vector machine (SVM), were implemented using RapidMiner. The dataset was stratified and divided into training (70%) and testing (30%) sets. Model performance was evaluated using accuracy, sensitivity, specificity, precision, and F1-score. Results: The Decision Tree demonstrated the best overall performance, with an accuracy of 90.00%, sensitivity (recall) of 89.23%, specificity of 92.00%, precision of 96.67%, and F1-score of 92.80%. The area under the ROC curve (AUC) for the Decision Tree model was 0.868, indicating good discriminative performance. Conclusion: The findings suggest that the decision tree model, using non-treatment-related clinical variables, may have potential as an auxiliary risk-stratification tool for RDS.
